      目的 观察结构化判读髋关节平扫MRI对于获得女性髋关节外偶然发现(IEHF)的价值。方法 回顾性分析1 052例女性患者髋关节MRI,依据修订后CT结肠成像报告数据系统将偶然发现(IF)分为4类,以其中E2~E4类为IEHF;与原始报告比较,分析IEHF检出率,观察不同年龄段IEHF分类的差异。结果 1 052例中,通过结构化读片于702例(702/1 052,66.73%)中检出1 198处IEHF,196处为原始报告所提及、1 002处(1 002/1 198,83.64%)原始报告未提及。其中980处(980/1 198,81.80%)为E2类IEHF,以宫颈腺囊肿最多(351/980,35.82%);96处(96/1 198,8.01%)为E3类,以卵巢大囊肿最常见(23/96,23.96%);122处(122/1 198,10.18%)为E4类,以腹股沟区淋巴结肿大最常见(32/122,26.23%)。1 002处原始报告未提及IEHF中,E2、E3及E4类占比分别为90.00%(882/980)、56.25%(54/96)及54.10%(66/122)。40岁以下、40~60岁、60岁以上患者IEHF均以E2类为主,但不同年龄段患者IEHF分类差异有统计学意义(P<0.001)。结论 结构化判读女性髋关节平扫MRI有助于获得女性IEHF。

      Objective To observe the value of structured imaging reading of hip joint MRI for detecting incidental extra-hip findings (IEHF) in female patients. Methods Data of 1 052 female patients who underwent MR scanning of hip joint were retrospectively analyzed. According to the revised CT colonography reporting and data system, the incidental findings (IF) were divided into E1—4 types, and E2 to E4 type IF were regarded as IEHF. The detection rate of IEHF was analyzed and compared with the original report. The classifications of IEHF were compared among patients in different ages. Results Totally 1 198 IEHF were detected through structured imaging reading in 702 patients (702/1 052, 66.73%), among them 196 IEHF were mentioned but 1 002 IEHF (1 002/1 198, 83.64%) were not mentioned in original reports. There were 980 E2 type IEHF (980/1 198, 81.80%), among which most were cervical cysts (351/980, 35.82%). There were 96 E3 type IEHF (96/1 198, 8.01%), most of which were large ovarian cyst (23/96, 23.96%). There were 122 E4 type IEHF (122/1 198, 10.18%), the most were enlarged lymph nodes in the inguinal region (32/122, 26.23%). The proportion of unreported E2, E3 and E4 type IEHF was 90.00% (882/980), 56.25% (54/96), 54.10% (66/122), respectively. E2 type IEHF was the most common in patients under 40 years old, 40—60 years old and over 60 years old, whereas significant difference of IEHF classification was found among patients with different ages (P<0.001). Conclusion Structural imaging reading of hip joint MRI was helpful for detecting IEHF in female patients.
