目的 观察胎儿心脏定量(Fetal HQ)技术评估宫内生长受限胎儿心脏大小、形态和功能的价值。方法 前瞻性纳入60胎生长受限[(FGR)组]及60胎正常胎儿(对照组)。采用Fetal HQ技术测量胎儿四腔心长径(4CVTL)和宽径(4CVTW),获得整体球形指数(GSI);以斑点追踪技术追踪左、右心室舒张期和收缩期心内膜运动轨迹,获取相关参数。比较各参数组间差异,评价其与胎龄的相关性。结果 FGR组4CVTL、4CVTW、四腔心面积(4CVA)、左心室舒张末期面积(LV-EDA)、每搏输出量(SV)及心输出量(CO)均小于对照组(P均<0.001)。FGR组4CVTL、4CVTW、4CVA、LV-EDA、右心室舒张末期面积(RV-EDA)及左心室整体纵向应变(LVGLS)均与胎龄呈正相关(rs=0.29~0.58,P均<0.05);左心室面积变化分数(LV-FAC)及左心室射血分数与胎龄均呈负相关(rs=-0.32、-0.32,P均<0.05)。对照组4CVTL、4CVTW、4CVA、LV-EDA、RV-EDA、SV、CO及LV-GLS均与胎龄呈正相关(rs=0.28~0.70,P均<0.05)。结论 Fetal HQ技术可定量评估宫内生长受限胎儿心脏大小、形态及功能。 |

Objective To observe the value of fetal heart quantification (Fetal HQ) for evaluation of heart size, morphology and function in fetuses with growth restriction. Methods Sixty fetuses with growth restriction (fetal growth restriction [FGR] group) and 60 healthy fetuses (control group) were prospectively enrolled. Fetal HQ technique was used to measure the length (4CVTL) and width (4CVTW) on four-chamber view, and the global spherical index (GSI) was obtained. The diastolic and systolic endocardial motion trajectories of left and right ventricular were tracked using speckle tracking technology, and the related parameters were obtained. Then parameters of fetal heart size, morphology and function were compared between groups, and the correlations of each parameter with gestational age were evaluated. Results 4CVTL, 4CVTW, four-chamber view area (4CVA), left ventricular end-diastolic area (LV-EDA), stroke volume (SV) and cardiac output (CO) in FGR group were smaller than those in control group (all P<0.001). In FGR group, 4CVTL, 4CVTW, 4CVA, LV-EDA, right ventricular end-diastolic area (RV-EDA) and left ventricular global longitudinal strain (LVGLS) were positively correlated with gestational age (rs=0.29—0.58, all P<0.05), while left ventricular fractional area change (LV-FAC) and left ventricular ejection fraction were negatively correlated with gestational age (rs=-0.32, -0.32, both P<0.05). In control group, 4CVTL, 4CVTW, 4CVA, LV-EDA, RV-EDA, SV, CO and LV-GLS were all positively correlated with gestational age (rs=0.28—0.70, all P<0.05). Conclusion Fetal HQ technology could be used to quantitatively analyze the size, morphology and function of heart in fetuses with intrauterine growth restriction. |
