      目的 观察剪切波弹性成像(SWE)定量评估不同程度酒精性脂肪性肝病(AFLD)患者颈总动脉弹性的价值。方法 纳入92例长期饮酒男性患者,将其分为A组(无AFLD,n=33)、B组(轻度AFLD,n=30)及C组(中重度AFLD,n=29);以32名不饮酒健康男性为对照组(D组)。比较4组一般资料、左侧颈总动脉常规超声及SWE参数,计算管壁运动度(△D)和动脉僵硬度系数(β);观察一般资料及常规超声参数与SWE参数的相关性,分析SWE定量评估颈总动脉弹性的价值。结果 相比A、B及D组,C组体质量指数、收缩压、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)明显升高(P均<0.05);相比A及D组,B、C组谷丙转氨酶(GPT)、谷草转氨酶(GOT)、甘油三酯(TG)均增高(P均<0.05),且C组均高于B组(P均<0.05);C组、B组、A组及D组γ-谷氨酰转移酶(GGT)依次减低(P均<0.05)。C组颈动脉内中膜厚度(CIMT)和β显著高于其他3组(P均<0.05);相比A及D组,B、C组最大弹性模量(MEmax),平均弹性模量(MEmean)、最小弹性模量(MEmin)均明显增高(P均<0.05),且C组均高于B组(P均<0.05)。长期饮酒者MEmax、MEmean、MEmin与GGT、GPT及β均呈正相关(r均>0.39,P均<0.01)。线性回归分析显示,长期饮酒者肝脏脂肪性变程度与SWE参数存在相关性(P均<0.05)。结论 SWE可定量评估不同程度AFLD患者颈总动脉弹性。

      Objective To observe the value of shear wave elastography (SWE) for quantitative evaluation on common carotid artery elasticity in patients with different degrees alcoholic fatty liver disease (AFLD). Methods Totally 92 male patients with long-term alcohol consumption were enrolled and divided into group A (without AFLD, n=33), group B (mild AFLD, n=30) or group C (moderate to severe AFLD, n=29), while 32 healthy men without history of alcohol consumption were selected as control group (group D). Then general information as well as conventional ultrasound and SWE parameters of left common carotid artery were compared among 4 groups, and the degree of motion of wall (△D) and arterial stiffness coefficient (β) were calculated. The correlations of general information and conventional ultrasound parameters with SWE parameters were analyzed. The value of SWE for quantitative evaluation on common carotid artery elasticity was evaluated. Results Compared with group A, B and D, body mass index, systolic blood pressure, serum total cholesterol (TC) and low density lipoprotein-cholesterol (LDL-C) of group C were significantly higher (all P<0.05). Compared with group A and D, glutamic-pyruvic transaminase (GPT), glutamic-oxaloacetic transaminase (GOT) and triglyceride (TG) in group B and C were all higher (all P<0.05), while those in group C were all higher than those in group B (all P<0.05). The γ-glutamyltransferase (GGT) decreased sequentially in group C, B, A and D (all P<0.05), respectively. Carotid intima-media thickness (CIMT) and β of group C were significantly higher than those of the other 3 groups (all P<0.05). Compared with group A and D, the maximum elastic modulus (MEmax), mean elastic modulus (MEmean) and minimum elastic modulus (MEmin) in group B and C were higher (all P<0.05), while those in group C were all higher than those in group B (all P<0.05). MEmax, MEmean and MEmin of patients with long-term alcohol consumption were positively correlated with GGT, ALT and β (all r>0.39, all P<0.01). Linear regression analysis showed significant correlations between the degrees of liver steatosis and SWE parameters in patients with long-term alcohol consumption (all P<0.05). Conclusion SWE could be used to quantitatively evaluate common carotid artery elasticity in patients with AFLD of different degrees.
