目的 采用一体化18F-FDG-PET/MRI观察帕金森病(PD)患者脑葡萄糖代谢与局部一致性(ReHo)的关系。方法 前瞻性纳入34例PD患者(PD组)及25名健康受试者(对照组),行颅脑一体化PET/MR检查;以独立样本t检验比较组间脑葡糖代谢标准摄取值比值(SUVR)及ReHo差异,通过Pearson相关性分析探讨SUVR与ReHo异常重叠脑区二者的关系,以及PD患者重叠脑区SUVR、ReHo与病程及临床量表评分的相关性。结果 相比对照组,PD组初级感觉运动区、中央旁小叶、壳核、苍白球及丘脑SUVR升高(P均<0.05,经FDR校正),视觉区、颞叶及顶叶SUVR降低(P均<0.05,经FDR校正);视觉区、颞叶及顶叶ReHo升高(P均<0.05,经FDR校正),中央旁小叶、壳核、苍白球、丘脑、脑干、额下回眶部及后扣带回ReHo降低(P均<0.05,经FDR校正)。SUVR与ReHo异常重叠脑区包括双侧颞下回、双侧枕中回、双侧壳核、双侧中央旁小叶及左侧距状裂;上述区域中,SUVR与ReHo组间差异有显著空间相似性(r=-0.84,P=0.005)。PD组与对照组组内SUVR与ReHo异常重叠脑区的SUVR平均值与ReHo平均值均无显著相关性(P均>0.05)。PD患者双侧壳核ReHo均与病程呈正相关(左侧:r=0.36,P=0.04;右侧:r=0.48,P<0.01),左侧距状裂SUVR与Hoehn-Yahr分级呈正相关(r=0.44,P=0.01)。结论 PD患者脑葡萄糖代谢与ReHo异常空间分布相似,但其间不存在线性相关关系。 |

Objective To observe the association between cerebral glucose metabolism and regional homogeneity (ReHo) in patients with Parkinson's disease (PD) using integrated 18F-FDG-PET/MRI. Methods Totally 34 PD patients (PD group) and 25 healthy controls (control group) were prospectively enrolled. Cerebral integrated PET/MR examination was performed, cerebral glucose metabolism standard uptake value ratio (SUVR) and ReHo were investigated and compared between groups with independent-sample t tests. Brain regions with both SUVR and ReHo abnormalities were selected, and the relationship of SUVR and ReHo abnormalities, the correlation between SUVR and ReHo in overlapping brain regions in PD patients with PD duration and clinical scale scores were analyzed using Pearson's correlation analysis. Results Compared with control group, increased SUVR was noticed in the primary sensorimotor cortex, paracentral lobule, putamen, pallidum and thalamus (all P<0.05, corrected by FDR), while decrease SUVR was found in the visual area, temporal area and parietal area in PD group(all P<0.05, corrected by FDR). Meanwhile, increased ReHo was observed in the visual area, temporal area and parietal area (all P<0.05, corrected by FDR), and decreased ReHo was obtained in the paracentral lobule, putamen, pallidum, thalamus, brainstem, orbital part of inferior frontal gyrus and posterior cingulate cortex in PD group (all P<0.05, corrected by FDR). The overlapping areas of both SUVR and ReHo abnormalities included bilateral inferior temporal lobes, bilateral middle occipital lobes, bilateral putamens, bilateral paracentral lobules and left calcarine fissure. There was significant spatial similarity in differences between SUVR and ReHo groups (r=-0.84, P=0.005). No significant correlation was found between the average SUVR and ReHo values of overlapping brain regions within PD nor control group (both P>0.05). In PD patients, ReHo of the bilateral putamen were positively correlated with PD duration (left: r=0.36, P=0.04; right:r=0.48, P<0.01), while SUVR of left calcarine fissure was positively correlated with Hoehn-Yahr stage (r=0.44, P=0.01). Conclusion The spatial abnormalities of glucose metabolism and ReHo of PD patients were similar, but no linear correlation was detected. |
