      目的 采用二维斑点追踪成像(2D-STI)技术评价红景天苷对脓毒症大鼠心功能的保护效果。方法 将32只无特定病原体(SPF)级12周龄SD大鼠随机均分为对照组、脓毒症组、低剂量组、高剂量组(n均=8)。对照组、脓毒症组以15 mg/kg体质量生理盐水连续灌胃7天,低、高剂量组分别以15 mg/kg、30 mg/kg体质量红景天苷连续灌胃7天。于末次灌胃后1 h予对照组10 mg/kg体质量生理盐水腹腔注射,余组予等体质量脂多糖腹腔注射。12 h后测量并比较各组心脏常规超声心动图及2D-STI参数差异,并检测下腔静脉血心肌肌钙蛋白Ⅰ(cTnⅠ)及脑钠肽(BNP);以Spearman相关性分析评价2D-STI参数与心肌指标的关系。结果 脓毒症组左心室短轴缩短率(LVFS)、射血分数(LVEF)、各层心肌整体圆周应变及各心肌节段圆周应变率(SrC)均明显低于对照组(P均<0.05),其血清cTnⅠ及BNP均明显高于对照组(P均<0.05)。低、高剂量组各参数及指标均不同程度升高;2D-STI参数中,左心室心内膜下层心肌圆周应变(GCS-Endo)与cTnI相关性最高(r=-0.875,P<0.05)。结论 2D-STI技术可用于评价红景天苷对脓毒症大鼠心肌损伤的保护作用;GCS-Endo是较为敏感的监测指标。

      Objective To observe the protective effect of salidroside on cardiac function of septic rats with two-dimensional speckle tracking imaging (2D-STI). Methods A total of 32 specific pathogen free (SPF) SD rats aged 12 weeks were randomly divided into control group, sepsis group, low dose group and high dose group (each n=8). Rats in control group and sepsis group were given 15 mg/kg body mass saline for 7 consecutive days, while those in low and high dose groups were given 15 mg/kg and 30 mg/kg salidroside for 7 consecutive days. One hour after the last gavage, rats in control group was given intraperitoneal injection of 10 mg/kg saline, and those in the other groups were given intraperitoneal injection of equal-weight lipopolysaccharide. Twelve hours later, conventional echocardiogram and 2D-STI parameters were measure and compared among groups, and cardiac troponin Ⅰ (cTnⅠ) and brain natriuretic peptide (BNP) of inferior vena cava blood were detected. Spearman correlation was used to analyze the relationships of 2D-STI parameters and myocardial indexes. Results The left ventricular fraction shortening (LVFS), left ventricular ejection fraction (LVEF), overall circumferential strain of each myocardium and circumferential strain rate (SrC) of each myocardial segment in sepsis group were all significantly lower than those in control group (all P<0.05). Serum cTnⅠ and BNP in sepsis group were significantly higher than those in control group (both P<0.05). Parameters and indexes in low and high dose groups increased to varied degrees. Among 2D-STI parameters, left ventricular global circumferential strain in subendocardial myocardium (GCS-Endo) had the highest correlation with cTnⅠ (r=-0.875, P<0.05). Conclusion 2D-STI technology could be used to evaluate the protective effect of salidroside on myocardial injury in septic rats. GCS-Endo was a sensitive monitoring index.
