      目的 定量测量CT图像中股骨后髁形态学参数,观察其形态学特征与非接触性前交叉韧带(ACL)断裂的相关性。方法 纳入179例非接触性ACL断裂患者膝关节CT图像(ACL断裂组),以182名健康志愿者作为对照组,定量测量并比较2组膝关节股骨后髁相关形态学参数,包括股骨髁内外侧宽度(MLW)、内侧后髁宽度(MPCW)、外侧后髁宽度(LPCW)、内侧髁长度(MCL)、外侧髁长度(LCL)、内侧后髁长度(MPCL)、外侧后髁长度(LPCL)、内侧后髁高度(MPCH)及外侧后髁高度(LPCH),计算LPCW/MLW、MPCW/MLW、LPCL/LCL和MPCL/MCL。采用组内相关系数(ICC)评估测量值的一致性。结果 观察者间及观察者内各参数测量值的一致性优异(ICC均≥0.80)。ACL断裂组MLW、MPCW、MCL、LCL、MPCL和MPCL/MCL均大于对照组(P均<0.05),LPCL/LCL小于对照组(P<0.05);组间内外侧后髁高度参数MPCH和LPCH差异均无统计学意义(P均>0.05)。结论 股骨远端后髁形态与非接触性ACL断裂存在相关性。

      Objective To quantitatively measure the morphological parameters of the posterior femoral condyle based on CT images, and to observe the correlations of morphological features and non-contact anterior cruciate ligament (ACL) ruptures. Methods A total of 179 patients with non-contact ACL rupture (ACL rupture group) with CT images of knee joints were included, and 182 healthy volunteers were taken as control group. The morphological parameters of femoral posterior condyle were measured, including the medial-lateral width of epicondyle (MLW), the medial posterior condyle width (MPCW), the lateral posterior condyle width (LPCW), the medial condyle length (MCL), the lateral condyle length (LCL), the medial posterior condyle length (MPCL), the lateral posterior condyle length (LPCL), the medial posterior condyle height (MPCH) and the lateral posterior condyle height (LPCH), while LPCW/MLW, MPCW/MLW, LPCL/LCL and MPCL/MCL were calculated and compared between groups. Intra-class correlation coefficient (ICC) was used to assess the consistency of the measured values. Results The measurements of all above parameters had excellent intra- and inter-observer consistency (all ICC ≥ 0.80). MLW, MPCW, MCL, LCL, MPCL and MPCL/MCL of ACL rupture group were higher than those of control group (all P<0.05), but LPCL/LCL in ACL rupture group was less than that in control group (P<0.05). There was no statistical difference of the height of the medial and lateral posterior condyles (MPCH and LPCH) between groups (both P>0.05). Conclusion Non-contact anterior cruciate ligament rupture was correlated with the morphology of distal femoral posterior condyle.
