      目的 探讨超声实时组织弹性成像评价兔腹主动脉粥样硬化斑块稳定性的价值。方法 将20只日本大耳白兔随机分为正常组和模型组,模型组以高脂喂养-免疫损伤法构建动脉粥样硬化模型。实验开始前及结束后检测血清总胆固醇(TC)、甘油三酯(TG)及低密度脂蛋白(LDL)含量,超声弹性成像检测斑块应变对比值(SR),检查结束后取病理标本行油红“O”染色及HE染色,构建ROC曲线分析斑块SR在评价斑块稳定性上的诊断价值。结果 造模前两组间血清TC、TG、LDL值差异无统计学意义(P>0.05),造模结束后模型组血清TC、TG、LDL水平高于正常组(P<0.05);模型组大体标本油红“O”染色示动脉管腔内大片红染的粥样斑块,病理切片HE染色见粥样斑块形成,造模成功率100%,不稳定斑块数量占60.00%(6/10);ROC曲线分析示超声SR值诊断不稳定斑块有较好的敏感度(83%)和特异度(75%),SR值诊断界值为2.22,曲线下面积为0.83(P<0.05)。结论 超声实时组织弹性成像SR值可用于斑块半定量分析,在评价动物粥样硬化斑块稳定性方面有较好的应用价值。

      Objective To explore the application value of ultrasound real-time tissue elasticity imaging in evaluating the stability of rabbit abdominal aorta atherosclerotic plaques. Methods Totally 20 Japanese white rabbits were randomly divided into normal group and model group. The animals in the model group were fed with high fat diet and accepted immunological injury. Before the start and after of the experiment, the level of serum total cholesterol (TC), triglyceride (TG) and low density lipoprotein (LDL) was detected; ultrasound elasticity imaging was taken to detect the plaque strain ratio (SR). The rabbits were sacrificed after the examination and pathological analysis was performed; ROC curve was constructed to analyze the diagnostic value of SR in evaluating the stability of plaque. Results Before the experiment, the differences of the serum TC, TG, LDL level between the two groups were not statistically significant (all P>0.05). After the experiment, the serum TC, TG, LDL levels of the model group were higher than those of the normal group (all P<0.05). Oil red O staining showed that the atherosclerotic plaque was stained red and the HE staining showed that the plaque was formatted in the model group. The success rate of the model was 100% and the unstable plaque rate accounted for 60.00% (6/10). ROC curve analysis showed that SR in diagnosis of unstable plaque had better sensitivity (83%) and specificity (75%). The diagnostic cut-off point of SR was 2.22 and the area under the curve was 0.83 (P<0.05). Conclusion Ultrasound real-time tissue elasticity imaging can be used for the semi quantitative analysis of the plaque, which has good application value in evaluating the stability of atherosclerotic plaque in animals.
