      目的 探讨能谱CT低浓度对比剂增强扫描在肺动脉成像中的临床应用价值。方法 体质量指数为22~23 kg/m2的150例患者分为A~E共5组并分别接受CT肺动脉造影,扫描采用能谱扫描模式,患者注射对比剂总量为0.4 ml/kg体质量。5组患者注射对比剂浓度分别为A组320 mgI/ml,B组256 mgI/ml,C组192 mgI/ml,D组160 mgI/ml,E组128 mgI/ml。扫描结束获得0.625 mm层厚的肺动脉最佳对比度噪声比(CNR)单能量keV图像,用此单能量keV图像对肺动脉行VR、MIP、MPR。测量5组图像中肺动脉和相同层面背部肌肉竖脊肌及胸壁脂肪的CT值及其标准差,计算SNR及CNR并取平均值,并由2名高年资医师盲法对5组图像的质量进行评分。结果 A~E组SRN分别为23.61±3.72、22.18±3.13、22.17±2.91、21.87±2.53、21.72±2.82(P均>0.05),CNR分别为26.35±4.12、26.08±3.79、25.53±3.82、24.85±3.22、24.68±3.56。A~E组图像质量的评分分别为4.53±0.81、4.45±0.78、4.46±0.75、4.38±0.79、4.41±0.57(P均>0.05)。2名医师对A~E组肺动脉图像质量评价的一致性好。结论 应用能谱CT扫描中的最佳单能量keV进行CTPA检查可显著降低对比剂使用浓度,推荐使用的对比剂浓度为128 mgI/ml,注射总量为0.4 ml/kg体质量。

      Objective To investigate the value of low concentration of contrast media with energy spectral CT on CT pulmonary angiography. Methods Totally 150 patients with body mass index from 22 kg/m2 to 23 kg/m2 underwent spectral CT and were divided into A-E groups. The contrast media concentration were 320 mgI/ml in A group, 256 mgI/ml in B group, 192 mgI/ml in C group, 160 mgI/ml in D group, 128 mgI/ml in E group. and the total contrast media were 0.4 ml/kg body weight. The 0.625 mm slice thickness of optimal monochromatic images were obtained, VR, MIP and MPR were created to show the pulmonary angiography respectively. The CT value and standard deviation were measured at the pulmonary angiography, the erector muscle of spine and the fat of chest wall, SNR and CNR were calculated. The overall imaging quality was evaluated by two radiologists. Results The SNR of A-E groups were 23.61±3.72, 22.18±3.13, 22.17±2.91, 21.87±2.53, 21.72±2.82 (all P>0.05) and the CNR was 26.35±4.12, 26.08±3.79, 25.53±3.82, 24.85±3.22, 24.68±3.56 (all P>0.05). Score of imaging quality of A-E groups were 4.53±0.81, 4.45±0.78, 4.46±0.75, 4.38±0.79, 4.41±0.57 (all P>0.05). Consistency of two radiologists in evaluation on image quality of 5 groups were good. Conclusion Contrast media concentration could be reduced in the best monochromatic images of spectral CT. Contrast media of 128 mgI/ml and injection total amount of 0.4ml/kg body weight are recommended.
