      目的 探讨经皮CEUS检测荷瘤兔乳腺癌转移前哨淋巴结(MSLN)的价值。方法 建立28只兔VX2乳腺癌模型,建模3周后对其行CEUS检查、99Tcm-SC淋巴闪烁照像和亚甲蓝示踪成像检测SLN,观察是否存在转移,以病理结果为金标准,计算并比较3种方法对SLN的检出率,并评价诊断效能。结果 CEUS、99Tcm-SC淋巴闪烁照像和亚甲蓝示踪对SLN检出率分别为80.56%(29/36)、86.11%(31/36)和77.78%(28/36),差异无统计学意义(P=0.956),诊断MSLN的敏感度、特异度、准确率分别为90.48%(19/21)、75.00%(6/8)、86.21%(25/29),92.31%(24/26)、40.00%(2/5)、83.87%(26/31)和87.50%(21/24)、75.00%(3/4)、85.71%(24/28),差异亦无统计学意义(P均>0.05)。结论 CEUS检测MSLN效能与99Tcm-SC淋巴闪烁照像和亚甲蓝示踪成像无明显差异,且无创、操作简便、价格低廉,临床前景广阔。

      Objective To investigate the value of percutaneous CEUS in detecting metastasis sentinel lymph node (MSLN) in breast cancer rabbit models. Methods Totally 28 rabbit models of VX2 breast cancer were successfully established and CEUS, 99Tcm-SC dynamic imaging, methylene blue dyeing were performed to detect the SLN and assess the metastasis of the SLN. The detection rate of SLN and the diagnostic efficacy were calculated and compared between three methods with pathological results as gold standard. Results The SLN detect rates of CEUS, 99Tcm-SC dynamic imaging, methylene blue dyeing was 80.56% (29/36), 86.11% (31/36) and 77.78% (28/36), respectively, with no significant difference (P=0.956). The sensitivity, specificity and accuracy of diagnosing MSLN was 90.48% (19/21), 75.00% (6/8), 86.21% (25/29) for CEUS, 92.31% (24/26), 40.00% (2/5), 83.87% (26/31) for 99Tcm-SC dynamic imaging and 87.50% (21/24), 75.00% (3/4), 85.71%(24/28) for methylene blue dye, with no significant difference (all P>0.05). Conclusion CEUS is noninvasive, easy to operate and inexpensive in detecting MSLN in breast cancer with the same diagnosis abilitiy compared with 99Tcm-SC dynamic imaging and methylene blue dyeing, which is more suitable for clinical application.
