      目的 观察阿尔茨海默病(AD)患者全脑三维(3D)伪连续动脉自旋标记(3DpcASL)成像中脑血流灌注(CBF)特点,分析其与认知功能评分的相关性。方法 对16例AD患者(AD组)和19例年龄匹配的认知功能正常者(对照组)进行常规MR和3DpcASL灌注成像。使用SPM8软件对所获脑血流图进行预处理,比较两组CBF差异,评价CBF减低脑区CBF值与MMSE、MoCA评分的相关性。结果 AD组CBF灌注减低区主要见于双侧颞上回、颞中回、颞下回、双侧楔前叶、双侧中扣带回、后扣带回、左侧角回、左侧顶下回及左侧额上回(t=5.32,P<0.001);双侧楔前叶、双侧中后扣带回、双侧颞上回、左侧颞中回、左侧颞下回、左侧角回及左侧顶下回CBF值与MMSE、MoCA评分呈正相关(P<0.05)。结论 3DpcASL可发现AD患者脑区灌注异常,灌注减低区的CBF值与认知功能减低存在相关性。

      Objective To evaluate cerebral blood flow (CBF) values in patients with Alzheimer disease (AD) using whole-brain 3D pseudocontinuous arterial spin labeling (pcASL), and to compare the correlation between CBF and cognitive function scores. Methods Sixteen AD patients (AD group) and 19 age-matched cognitively normal control subjects (control group) were recruited. 3D pcASL was performed. CBF maps were analyzed by using SPM 8. CBF was compared between two groups. Correlation of CBF with cognition (mini-mental state examination, MMSE; Montreal cognitive assessment, MoCA) was investigated. Results Compared with control group, hypoperfusion was found in bilateral superior temporal gyri, middle temporal gyri, inferior temporal gyri, precuneus, middle cingulum, posterior cingulum, left angular gyrus, left inferior parietal gyrus and left superior frontal gyrus in AD group. CBF values of hypoperfusion areas in bilateral precuneus, bilateral middle cingulum, posterior cingulum, left middle temporal gyri, left inferior temporal gyri, left angular gyrus, left inferior parietal gyrus were positive correlated with scores of MMSE and MoCA (P<0.05). Conclusion Regional hypoperfusion areas could be observed in AD patients using 3D pcASL, which positively correlated with the decline of cognitive function.
