      目的 分析军训致下肢早期应力性骨损伤的64排螺旋CT和MRI表现,探讨二者对军训致下肢早期应力性骨损伤的诊断价值。方法 回顾性分析35例经临床和影像证实的军训致下肢早期应力性骨损伤病例的影像学资料,35例均行直接数字化摄片(DR)、64排螺旋CT及MR检查。结果 35例患者中,33例为单侧,2例为双侧损伤,共37处应力性损伤,其中股骨5例、胫骨24例、跖骨6例,双侧胫骨均受累2例;X线平片检出7处病变,64排螺旋CT检出19处病变,MRI检出全部37处病变;三种方法比较,差异有统计学意义(χ2=50.45、23.79、8.54,P均<0.01)。结论 MRI能很好地显示军训致下肢早期应力性骨损伤的各种征象,明显优于X线平片和64排螺旋CT,是诊断早期应力性骨损伤的最佳方法;64排螺旋CT可以发现早期应力性骨损伤的细微骨折线及骨皮质内的异常密度改变,二者结合对于军训致下肢早期应力性骨损伤具有很高的临床应用价值。
英文摘要:
      Objective To analyze 64- slice spiral CT and MRI findings of early lower extremity stress injury caused by military training. and investigate clinical value of the two methods to the injury. Methods Plain films, 64- slice spiral CT and MR imaging data of 35 patients with early lower extremity stress injury proved clinically were reviewed. Results Totally 35 patients (37 lesions) were diagnosed, including 33 cases with unilateral lesions and 2 cases with bilateral lesions. Five cases were located in femur, 24 cases in tibia and 6 cases in metatarsal. Totally 7, 19, 37 lesions were diagnosed with plain films, 64- slice spiral CT and MRI, respectively. The differences between above every two modalities were significant (χ2=50.45, 23.79, 8.54, P<0.01). Conclusion Early lower extremity stress injury caused by military training can showed clearly with MRI, which is superior to plain films and 64- slice spiral CT significantly. MRI is the best method in diagnosing early lower extremity stress injury. Microfracture lines and density abnormalities of the cortical bone can be showed with 64- slice spiral CT. MRI combined with 64- slice spiral CT have great clinical value in diagnosing early lower extremity stress injury caused by military training.
