      目的 评价椎基底动脉扩张纡曲症(VBD)影像学最佳检查方法及其影像学表现。方法 分析64例出现中枢神经系统症状VBD患者的各种影像学资料,均接受MR平扫,59例同时接受磁共振血管造影(MRA),45例接受螺旋CT扫描,其中31例接受CT血管造影(CTA)和三维重建(MIP、SSD、MPR),6例接受数字减影血管造影(DSA)检查。对各种影像结果进行分析比较。结果 颅神经受扩张和纡曲的椎基底动脉推压,以Ⅷ、Ⅵ及Ⅴ对颅神经受累为主。39例CT检查可见椎基底动脉血管壁钙化斑,22例合并小脑和(或)脑干的梗死。结论 MR及MRA对VBD的检出明显优于CT,可作为基底动脉延长纡曲症诊断和鉴别诊断的首选影像学方法。
英文摘要:
      Objective To assess the appropriate examination method and imaging manifestation of vertebrobasilar dolichoectasia (VBD). Methods The imaging data of 64 VBD patients with central nervous system symptoms were analyzed. All patients underwent MR scan, 59 patients underwent magnetic resonance angiography (MRA) at the same time, 45 patients underwent spiral CT scan, 31 patients underwent cerebral angiography (CTA) and three-dimensional reconstruction (MIP, SSD, MPR), 6 patients underwent digital subtraction angiography (DSA) examination. The results of imaging were compared. Results All patients had vertebrobasilar artery expansion, extension and tortuous. CT examination revealed that 39 patients with vertebral-basilar arterial wall calcification, 22 patients with cerebellar and (or) brainstem infarction. Conclusion MRI and MRA are better than helical CT scan in finding of the basilar artery extending, and can be used as preferred imaging method in diagnosis and differential diagnosis of VBD.
