       目的 探讨超声造影检测兔急性肾衰竭(ARF)肾皮质血流灌注变化及其与血肌酐(SCr)、尿素氮(BUN)的关系。方法 家兔后腿肌注50%甘油建立急性肾功能衰竭动物模型,分别于造模前(T0)、造模后1、4、8、12天(T1、T4、T8、T12)共5个时间点行肾灰阶超声造影检查,分析肾皮质回声的时间-强度曲线(TIC),参数包括造影剂到达时间(AT)、达峰时间(TTP)、峰值强度变化(A)、曲线上升支斜率(β),同时测各时间点SCr及BUN,分析其与超声造影各参数的相关性。结果 造模后曲线TTP、A和β在T1、T4、T8与造模前相比差异均有统计学意义(P<0.05),T1、T4、T8时间点各参数两两比较,差异不同。AT、TTP、A、β与血肌酐、尿素氮间不存在直线相关关系 (P>0.05)。结论 灰阶超声造影可较早反映ARF时肾皮质血流灌注的改变,但曲线各参数变化与SCr、BUN不存在直线相关关系。

      Objective To evaluate the renal cortical blood perfusion changes in rabbits with acute renal failure (ARF) with gray scale contrast-enhanced ultrasound, and to explore the relationship between these changes and the blood creatinine (SCr), as well as the blood urea nitrogen (BUN). Methods Rabbit ARF models were established with 50% glycerin injected into the rabbits thighs. Gray scale contrast-enhanced ultrasound was performed on the day before injection (T0) and 1, 4, 8, 12 days (T1, T4, T8, T12) after injection. The renal cortex perfusion time-intensity curve (TIC) was analyzed, including parameters like arrival time (AT), time to peak intensity (TTP), amplitude of peak intensity (A) and slope rate of TIC (β) of renal cortex. Meanwhile the SCr and BUN were measured, the correlation between SCr, BUN and parameters were analyzed. Results Compared with the value of T0, the value of TTP, A, β after injection (T1, T4, T8) were statistically different, respectively (P<0.05), but the differences among T1 , T4 and T8 were various. No linear correlation between above parameters and SCr, BUN was found. Conclusion The renal cortical blood perfusion changes can be early observed with gray scale contrast-enhanced ultrasound, but there is no linear correlation between the changes of parameters and SCr, BUN.
