| 目的 探讨99Tcm-硫胶体前哨淋巴结显像、手持γ探测仪及高频超声联合使用在早期乳腺癌中前哨淋巴结的识别、定性准确性,临床可行性和实用价值。方法 对39例早期乳腺癌患者行SPECT 前哨淋巴结显像、高频超声检查及术中用手持γ探测仪探测前哨淋巴结,观察其位置、性质。结果 39例患者前哨淋巴结显像及γ探测仪检出前哨淋巴结的检出率分别为97.44%、 94.87%。超声发现所有发生转移的腋窝淋巴结的横径为0.6 cm以上,纵径为0.8 cm以上, 1.0≤纵横比＜2.2。三者的联合使用诊断早期乳腺癌前哨淋巴结定性的灵敏度、特异性、准确性分别为93.33%、91.67%、92.3%。结论 99Tcm-硫胶体行早期乳腺癌前哨淋巴结检测具有较高的检出率和指向性;联合检查是较早发现乳腺癌前哨淋巴结及非前哨淋巴结转移的一种有效而准确的方法。 |

| Objective To evaluate the clinical feasibility and the value of the combination of 99Tcm- sulfur colloid (99Tcm -SC) on sentinel lymph node (SLN) imaging with gamma probe SLN detection and ultrasonography in patients with early-stage breast cancer. Methods Thirty-nine women with early-stage breast cancer were accepted SPECT SLN, ultrasonography and gamma probe SLN detection.Results In 38/39 patients (97.44%), SLNs were detected with 99Tcm -SC lymphoscintigraphy. With the application of intraoperative gamma probe, SLN was detected in 37/39 patients (94.87%). Through analysis of the ROC curve in ultrasonography, the transverse and longitudinal diameter of the metastatic lymph nodes were more than 0.6 cm and 0.8 cm, 1.0≤L/B (the ratio of the longitudinal and transverse diameter) ＜2.2. The sensitivity, specificity and accuracy of the combination of 99Tcm -SC on SLN identification with gamma probe SLN detection and ultrasonography were 93.33%, 91.67% and 92.3%, respectively.Conclusion ①99Tcm -SC has a better detection rate and guide value in SLN detection of the early-stage breast cancer; ②It is an effective and exact method to combinate 99Tcm-SC SLN detection by lymphoscintigraphy with intraoperative gamma probe and ultrasonography in determing earlier SLN and non SLN metastasis in breast cancer patients. |
