| 目的 用统计学方法比较多层螺旋CT 与MRI 在胰头癌诊断与鉴别诊断中的价值。方法 顺序抽取2002 年9月- 2005 年3 月间经手术病理证实为壶腹周围病变,且在术前一周之内经本院多层螺旋CT 与MRI 检查的病例共62 例,包括胰头癌27 例,壶腹癌25 例,胆总管下段癌4 例,胰头炎症与胆总管下段结石各3 例。将62 例病例分为胰头癌组(27例) 和非胰头癌组(35 例) ,由两位放射科医师独立进行阅片,其结果分别进行Kappa 一致性检验和ROC 曲线分析。结果 ①两位医师判读结果经Kappa 一致性检验,Kappa 值分别为0. 828 和0. 894 , P > 0. 05 , 无显著性差异,说明两位医师具有良好的一致性。②取其中一位医师的CT 与MRI 判读结果进行ROC 曲线分析,Az 值分别为0. 880 与0. 863 。结论 由于多层螺旋CT 可以进行多期扫描,以及各种重建技术,大大提高了其对胰头癌诊断的准确性,其与MRI 互相补充,可以进一步提高诊断准确性。 |

| Objective To compare multi-slice CT (MSCT) and MRI in the diagnosis and differentiation of pancreatic headcarcinoma. Methods 62 cases of peri-ampullae disease were examined by MSCT and MRI One week pre-operation , f romSeptember 2002 to March 2005. These cases included carcinoma of the pancreatic head ( n = 27) , carcinoma of ampullae ( n =25) , carcinoma of distal bile duct (n = 4) , inflammation of pancreatic head ( n = 3) and stone of distal bile duct ( n = 3) . Thepatient s were divided into two groups (pancreatic head carcinoma and non-pancreatic head carcinoma) . The images were ana-lyzed separately by two radiologists , and the results were tested statistically by Kappa-test and ROC curve analysis. Results①The Kappa value of the analysis result s of two radiologist s were 0. 828 and 0. 894 , respectively ( P > 0. 05) . ②The Azvalue of ROC curve of MSCT and MRI were 0. 880 and 0. 863 , respectively. Conclusion The diagnostic accuracy of carcino-ma of pancreatic head was improved with multi-phase scan and reconst ruction technique of MSCT. The confidence of diagno-sis was enhanced by MSCT plus MRI. |
