      目的利用DTI及纤维追踪技术对语言功能区神经纤维分析,试图揭示原发性进行性失语主要白质结构的改变。方法 男,56岁,语言能力进行性下降3年。SIEMENS Trio 2003T完成全脑MRI和DTI数据收集和后处理。结果 常规MRI额叶、颞叶轻度萎缩。DTI左Broca区与其他脑区间纤维联系减少,三角区与Wernicke区无纤维联络。左侧Wernicke通过弓状纤维到达额叶岛盖部的纤维减少。Broca及Wernicke区平均FA及纤维束较正常人减少。结论 原发性进行性失语患者的语言功能区的纤维减少,脑内语言功能区纤维结构及其与其他脑区间纤维联系是语言功能完成的结构基础,尤其是前后语言功能区之间的弓状纤维束。

      Objective To exhibit the fibers of language areas, and to demonstrate fiber changes of the primary progressive aphasia with diffusion tensor imaging and fiber tracking technique.Methods Conventional MRI, diffusion tensor imaging (DTI) and fiber tracking were performed in a 56-year-old man with primary progressive aphasia by 3.0 T SIEMENS Trio 2003T MRI. FA values in language areas, including Broca's and Wernicke's areas, and contralateral normal areas were measured as seeds to track fibers. DTI method depicted the fibers in left Broca area and Wernicke area. Results Conventional MRI demonstrated mild atrophy of left temporal lobe and frontal lobe. The fasciculus between left Broca area and other areas decreased. There were no fiber connections between triangle and Wernicke's area, and fibers through arcuate fasciculus to opercula of frontal lobe decreased, too. Mean FA and neural fasciculi in Broca area and Wernicke area also decreased than those in healthy subjects.Conclusion The fibers between Wernicke area and other areas decreased. The fasciculi between cerebral language area and other areas are the basic structure of language function, especially the arcuate fasciculi are the most important structure between Broca and Wernicke's areas.
