      目的 评价MRI和31PMRS对脂质代谢性肌病的诊断价值。方法 收集经临床病理诊断的脂质沉积性肌肉病共8例。选择年龄相当的8例健康人作为对照组。评价其影像表现,并在31PMRS谱线上测量PCr、PP、ADP、Pi、PH的值,并计算Pi/PCr、Pi/ATP和PCr/ATP的比值。结果 在MRI T1WI上的异常形态改变可分为二种类型:①皮下、肌肉间及肌束间脂肪增多,为6例;②皮下、肌肉间脂肪明显减少,为2例。T2WI及压脂T2WI上肌肉未见异常信号。1例患者治疗前、后MRI影像表现未见变化。31PMRS表现为患者的PCr、PCr/ATP和PP较对照组明显降低(P<0.05),ADP较对照组明显增高(P<0.05),Pi、PH、Pi/PCr和Pi/ATP与对照组无明显差异(P>0.05)。结论 对于脂质沉积性肌肉病,MRI和31PMRS具有重要的应用价值。MRI可显示肌肉的形态改变,31PMRS可无创性地显示肌肉的代谢改变。

      Objective To evaluate the diagnostic value of MR imaging and 31P magnetic resonance spectroscopy (31PMRS) in patients with lipid storage myopathy (LSM). Methods Eight patients with pathologically proved LSM were enrolled in this study. Eight healthy volunteers were recruited to compose the control group. The MRI appearances were evaluated. The values of PCr, PP, ADP, Pi, PH were measured, and the ratios of Pi/PCr, Pi/ATP and PCr/ATP were calculated using the 31PMRS spectrum. Results There were 2 types of the abnormalities in MRI T1WI: ① increased adipose tissue could be found within the subcutaneous tissue, the intermuscular and intramuscular spaces in 6 cases; ② absence of normal adipose tissue within the subcutaneous tissue and the intermuscular spaces in 2 cases. As the results of 31PMRS, the average value of PCr, PCr/ATP and PP were significantly lower than that of the control group (P<0.05), whereas the average ADP value was significantly higher than that of the control group (P<0.05). There was not significantly different between the 2 groups concerning the average values of Pi, PH, Pi/PCr and Pi/ATP (P>0.05). Conclusion MR imaging and 31PMRS had important significance in the diagnosis of LSM. MRI could reveal the morphological changes and 31PMRS could display the metabolic variations of the muscles noninvasively.
