      目的:探讨超声造影到达时间参数成像对胆囊病变的鉴别诊断价值。 方法:回顾性分析在我院行胆囊超声造影检查的患者。采用到达时间参数成像分析超声造影图像。评估灌注动脉模式,分析到达时间参数,包括病灶到达时间、同深度肝组织到达时间、病灶与同深度肝组织的到达时间差(△T)。 结果:共47例患者进入研究,胆囊癌25例,胆囊良性病变22例。到达时间参数成像示良、恶性病变的灌注动脉模式差异有统计学意义(P<0.001)。分析到达时间参数,恶性和良性病变的△T差异有统计学意义(-2.69±1.37 s和-0.21±1.37 s,p<0.001),即胆囊癌先于周围肝组织增强,而良性病变与肝脏同步增强。应用受试者工作特征曲线,在△T截止值为-1.05秒时,诊断的敏感性、特异性、阳性预测值和阴性预测值分别为81.8%、92.0%、85.2%、90.0%,诊断准确度87.2%。 结论:超声造影到达时间参数成像有助于胆囊病变的鉴别诊断。

      Purpose: The aim of this study was to evaluate the value of arrival time parametric imaging for differential diagnosis of gallbladder lesions. Methods: Patients with cholecystectomy who received contrast-enhanced ultrasonography (CEUS) were included in this study. Following CEUS, a software of the arrival-time parametric imaging (ATPI) system was used to analyze the video footage. The artery patterns of the lesions on ATPI in artery phase were analyzed. The quantitative parameters including arrival time of gallbladder lesions, arrival time of surrounding liver, the time difference (△T) were calculated. Results: A total of 47 patients (with 25 gallbladder carcinomas and 22 benign gallbladder lesions) were enrolled in this study. Arrival time parametric image showed the artery patterns were different in benign and malignant lesions of gallbladder in artery phase (P < 0.001). The malignant lesions presented more branched and irregular artery pattern, while benign lesions presented more scattered dot and linear artery pattern. The arrival time of the gallbladder lesions is not statistically significant. While arrival time difference (sec) of lesions and the surrounding liver (△T) in malignant and benign lesions were statistically significant (-2.69 ± 1.37 versus -0.21 ± 1.37; P < 0.001). The malignant lesions presented enhancement prior to the surrounding liver while the benign lesions presented a synchronous enhancement with the liver. At a △T cutoff value of -1.05 seconds (using the receiver operating characteristic curve), the sensitivity, specificity, positive predictive value and negative predictive value in differentiating gallbladder carcinoma from benign lesions were 81.8%, 92.0%, 85.2% and 90.0%, respectively. The diagnostic accuracy was 87.2%. Conclusion: Artery patterns and parameters of arrival-time parametric imaging using CEUS could provide more information for the differential diagnosis of gallbladder lesions.
