目的 利用CT容积扫描及三维重建等技术,建立上气道模型,探讨肥大腺样体与鼻咽腔容积关系。方法 收集109例1-10岁儿童鼻咽部CT容积扫描图像,测量A/N值、鼻咽腔容积等,根据A/N值将研究对象分为正常组和肥大组,采用t检验和χ2卡方检验比较各指标组间差异,Pearson相关分析腺样体大小与鼻咽腔容积的相关性。结果 肥大组鼻后间隙、鼻咽腔容积及鼻咽腔最狭窄处气道面积均较正常组小(P<0.01)。5岁以下儿童A/N值与鼻咽腔容积的相关系数小于6-10岁儿童(-0.43 vs. -0.49)。患阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患儿者A/N值与鼻咽腔容积相关系数大于非OSAHS者(-0.60 vs. -0.33)。结论 腺样体大小与鼻咽腔容积存在负相关关系,这种相关性大小存在年龄差异,患OSAHS儿童与非不患OSAHS者相比,腺样体大小与鼻咽腔容积相关性更大。 |

Objective To explore the relationship between the volume of nasopharynx cavity and adenoid hypertrophy by using CT volume scanning, three-dimensional reconstruction and establishing an upper airway model. Methods 109 children aged 1-10 years were collected for CT volume scanning of nasopharynx, and A/N value, and nasopharynx volume were measured. According to A/N, the subjects were divided into normal group and hypertrophic group. T-test and chi square test were used to compare the differences. Pearson correlation was used to analyses the correlation of adenoid size and nasopharynx volume. Results The postnasal space, nasopharynx volume and narrowest area of hypertrophic group were smaller than those of normal group (P < 0.01). The correlation coefficient between A/N and nasopharynx volume was -0.43 (P < 0.01) for children <5 years old and -0.49 (P<0.01) for children aged 6-10 years. The correlation coefficient between A/N and nasopharynx volume in patients with OSAHS was higher than that in patients without OSAHS (- 0.60 vs. - 0.33). Conclusion There is a negative correlation between the size of adenoids and the nasopharynx volume. There is an age difference in the correlation between the size of adenoids and the nasopharynx volume. And this correlation was greater in children with OSAHS than in children without OSAHS. |
