目的 探讨实时二维剪切波弹性成像( two‐dimensional shear wave elastography,2D SWE)技术在检测大鼠急性肝衰竭以及预防性用药治疗中的应用价值。方法 将雄性SD大鼠随机分为4组:对照组、模型组、乳果糖组和美常安组。实验第1至8天,美常安组予以美常安稀释后灌胃治疗,乳果糖组予以乳果糖灌胃治疗,对照组和模型组予以等量生理盐水。实验第6、7天,模型组、乳果糖组和美常安组腹腔注射TAA,对照组代之以生理盐水。实验第8天,采用2D SWE技术测量四组大鼠肝脏硬度值(LSM,单位Kpa)。构建受试者工作(ROC)曲线,根据曲线下面积(AUC)选出准确性最高的LSM进行组间比较。实验第9天,取大鼠肝组织行病理检查。 结果 模型组LSM所构建的ROC曲线中,平均值诊断效能最佳;以8.44kpa为截断值,其AUC、敏感度和特异度分别为0.768、100%和62.5%。模型组LSM较其他三组明显升高,差异有统计学意义(P<0.05)。结论 急性肝衰竭大鼠LSM高于对照组和治疗组,2D SWE技术对急性肝衰竭的检测及预防性用药治疗后的评估具有一定的应用价值。 |

Objective To investigate the application value of two‐dimensional shear wave elastography(2D SWE)in the detection of acute liver failure and the evaluation of prophylactic drug use in rats. Methods Male SD rats were randomly divided into four groups: control group , model group, lactulose group and Medilac-s group. From the first to the eighth day of the experiment, the Medilac-s group was treated by intragastric administration with Medilac-s after?dilution, the lactulose group was treated with lactulose, and the control group and the model group were given the same amount of normal saline. On the 6th and 7th day of the experiment, the model group, lactulose group and Medilac-s group were intraperitoneally injected with TAA, and the control group was replaced with normal saline. On the 8th day of the experiment, 2D SWE technique was used to detect the liver stiffness measurement(LSM, Kpa) in four groups of rats.?The receiver operating characteristic(ROC) curve was constructed, and the most accurate LSM value was selected according to the area under the curve(AUC)used for inter-group comparison.?On the 9th day of the experiment,the liver tissue in rats was taken for pathological examination. Results Among the ROC curves constructed by the LSM value of the model group, the diagnostic efficiency of LSMmean was the best. With cut-off value of 8.44Kpa, the AUC, sensitivity and specificity were 0.768, 100% and 62.5%. Compare with the other three groups ,the LSMmean in the model group was significantly higher than that in (P<0.05). Conclusion The liver stiffness measurement of rats with acute liver failure was higher than that of the control group and the treatment group. The LSM detected by 2D SWE technique has certain application value in the evaluation of acute liver failure and prophylactic drug use. |
