| 目的 探讨心尖局部形态与动力学特征在心尖肥厚型心肌病(AHCM)影像学诊断中的应用价值。方法 选择2015年8月至2018年8月AHCM患者51例,包括21例早期AHCM(P-AHCM组)和30例典型AHCM(T-AHCM组)患者。以40例健康志愿者(NC组)及44例单纯高血压左心室重构患者(HT组)作为对照。超声重点观察舒张末和收缩末心尖顶角(θap)及其收缩期变化率(△θap)、收缩期心尖射血速度(Vap)及其与左室流出道射血速度的比值(速度指数Vap/VLVOT)等。结果 相比NC组及HT组,P-AHCM组和T-AHCM组的qap均变小(P<0.05), △qap、Vap增大(均P<0.05)。结论 超声心动图可以检出AHCM患者甚至在早期阶段就已出现心尖顶角减小、局部射血速度及收缩期角度变化率增高,这些参数结合特异性心电图改变有望提高超声对AHCM的检出率。 |

| Objective To investigate the value of apical vertical angle and ejection velocity in the detection of apical hypertrophic cardiomyopathy(AHCM). Methods A total of 51 consecutive outpatients with AHCM,including 30 typical AHCM(T-AHCM)and 21 pre-AHCM (P-AHCM) patients were enrolled from August 2015 to August 2018. Forty matched healthy volunteers and forty-four hypertensives were recruited as comparator groups respectively (normal control and hypertension group,NC and HT group). All subjects underwent full echocardiographic studies to acquire and analyze apical vertical angle (qap) and its systolic rate of change (Δθap), peak ejection velocity (Vap) and its ratio to the velocity in left ventricular outflow tract (VLVOT). Results Compared with NC and HT groups, θap decreased and △qap and Vap increased significantly in both P-AHCM and T-AHCM groups [all P＜0.001]. Conclusion Decreased apical vertical angle and enhanced dynamics such as systolic rate of angle change and local ejection velocity were found in patients with AHCM even in its early stage, which might be helpful in the detection of AHCM. |
