      [摘要] 目的:探讨超微血管成像(SMI)技术结合2017年美国放射学会甲状腺影像报告和数据系统(ACR-TIRADS)在甲状腺良恶性结节中的诊断价值。 方法:常规超声根据ACR-TIRADS将甲状腺187个结节分为TR 1~5类,然后结合SMI技术重新调整ACR-TIRADS分类。通过ROC曲线确定最佳诊断界点,比较两种方法的诊断效能。 结果:校正前TR 1~5各类别实际恶性率分别为0.00%、0.00%、5.00%、37.74%、64.29%。经SMI校正后分别为0.00%、0.00%、5.56%、23.26%、66.36%。校正前敏感性、特异性、阳性预测值、阴性预测值及准确度分别为75.00%、66.02%、64.29%、76.40%及70.05%,经SMI校正后分别为86.91%、64.08%、66.36%、85.71%及74.33%。 结论:SMI技术能显示甲状腺结节内的穿支血流,与ACR-TIRADS的结合能提高甲状腺结节的诊断效能。 [关键词]:甲状腺结节; ACR-TIRADS; SMI技术

      [Abstract] Objetive To explore the diagnostic value of superb micro-vascular imaging (SMI) combined with the 2017 American Radiological Society Thyroid Imaging Reporting and Data System (ACR-TIRADS) in benign and malignant thyroid nodules. Methods A total of 187 thyroid nodules were classified into TR types 1 to 5 according to ACR-TIRADS on conventional ultrasound, and then the ACR-TIRADS classification was re-adjusted in combination with SMI technology. The optimal diagnostic boundary was determined by the ROC curve, and the diagnostic efficacy of the two methods was compared. Results The actual malignant rates of TR 1 to 5 before correction were 0.00%, 0.00%, 5.00%, 37.74%, and 64.29%, respectively. After correction with SMI technology, they were 0.00%, 0.00%, 5.56%, 23.26%, and 66.36%, respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy before and after correction with SMI technology were 75.00% and 86.91%, 66.02%and 64.08%, 64.29% and 66.36%, 76.40% and 85.71% ,70.05%and 74.33%, respectively. Conclusion SMI technology can show perforating blood flow in thyroid nodules, and the combination with ACR-TIRADS can improve the diagnostic efficacy of thyroid nodules.
