[摘 要] 目的 探讨DCE-MRI定量参数在儿童先天性胆总管囊肿所致肝纤维化中的应用价值。方法 纳入肝纤维化患儿33例,正常对照组14例。经肝脏DCE-MRI扫描后,拟合Tofts模型获得参数Ktrans、Kep及Ve。分析不同肝纤维化分期的定量参数差异及其相关性,ROC曲线分析各参数诊断肝纤维化的曲线下面积和最佳截界值。P <0.05为差异有统计学意义。结果 不同纤维化亚组的Ktrans、Kep具有统计学差异(P <0.05),且随肝纤维化程度增加而下降(r =-0.764,-0.720,P <0.05)。Ktrans、Kep判别正常vs肝纤维化和轻度vs重度肝纤维化的ROC曲线下面积分别为0.949、0.748和0.933、0.731,截界值分别为0.239、0.186和1.814 、1.663。结论 DCE-MRI定量灌注参数Ktrans、Kep可用于儿童CCC所致肝纤维化诊断并分期。 |

[Abstract] Objective To investigate the value of DCE-MRI quantitative parameters in liver fibrosis caused by congenital choledochal cyst(CCC) in children. Methods 33 patients with liver fibrosis were included and 14 healthy volunteers were served as a control group. All subjects were performed liver DCE-MRI and acquired quantitative parameters by Tofts model, including Ktrans, Kep and Ve. Statistical analyses were performed to evaluate differences in MR parameters of different liver fibrosis stages and their correlations, and the area under curve(AUC) of each parameter in diagnosing different grades of liver fibrosis,P <0.05 statistically significant. Results Ktrans and Kep were significantly different among all fibrosis groups(P <0.05) and decreased as liver fibrosis progressed (r =-0.764,-0.720,both P <0.05). AUC for identifying normal vs fibrosis and mild vs advanced fibrosis were 0.949 and 0.748 respectively by Ktrans. The cutoff values were 0.239 and 0.186. And the AUC for identifying normal vs fibrosis and mild vs advanced fibrosis were 0.933 and 0.731 respectively by Kep. The cutoff values were 1.814 and 1.663. Conclusion Perfusion parameters such as Ktrans and Kep may be valuable for detecting and staging liver fibrosis caused by CCC in children. |
