      目的 探讨不同经胸肺部超声检查方法(TLUS)在诊断间质性肺疾病(ILD)中的可行性。方法 收集本院呼吸内科、免疫内科住院的临床诊断为ILD的病人40例,所有患者均分别采用简化的前侧后胸壁14肋间隙扫查法、前侧胸壁28 肋间隙扫查法、前侧后胸壁72肋间隙扫查法三种方法对肺部进行扫查。计算每种超声检查方法的可重复性、一致性及扫查时间。结果 (1)同一医师/不同医师利用三种扫查方法对B线评价的可重复性及一致性均较高,其中同一医师利用14肋间隙扫查法的可重复性最高(ICC=0.9891,95%CI[0.9772,0.9948])、一致性最高(界限宽度=0.57±2.79,95%CI[-0.47,1.61]);不同医师利用14肋间隙扫查法的可重复性最高(ICC=0.9795,95%CI[0.9575,0.9902])、一致性最高(界限宽度=0.70±3.77,95%CI[-0.71,2.11])(2)同一/不同医师利用14肋间隙扫查法观察胸膜线严重程度的一致性高(Kappa值分别为0.948和0.895,P<0.01)。(3)三种扫查方法的扫查时间对比,14肋间隙扫查法最短,72肋间隙扫查法最长(P<0.01)。结论 利用简化的14肋间隙扫查法、28肋间隙扫查法和72肋间隙扫查法诊断ILD的可行性均较高,其中14肋间隙扫查法可重复性及一致性最高,且用时最短,临床中可以14肋间隙扫查法优先使用。

      Objective To investigate the role of different transthoracic lung ultrasound(TLUS)in the diagnosis of interstitial lung disease(ILD). Method Totally 40 patients with ILD in Respiratory Department or Immunology Department were selected. All patients were examined separately by the simplified 14 intercostals space method, the 28 intercostals space method and 72 intercostals space method. The repeatability, consistency and examination time of each ultrasonography method were calculated. Results (1)The intra-/inter- physicians repeatability and consistency of three ultrasonography methods in evaluating B line were high. The intra-physicians repeatability and consistency of measuring the B lines by 14 intercostals space method were the highest (ICC=0.9891, 95%CI[0.9772,0.9948 ]), (boundary width=0.57± 2.79,95%CI[-0.47,1.61]). The inter-physicians repeatability and consistency of measuring the B lines by 14 intercostals space method were the highest (ICC=0.9795,95%CI[0.9575,0.9902]),(boundary width =0.70±3.77,95%CI[-0.71,2.11]). (2) The intra-/inter- physicians consistency of pleural line observation by 14 intercostals space method was high (Kappa value was 0.948 and 0.895 respectively,P<0.01). (3)The examination time of three ultrasonography methods were different, the 14 intercostals space method was the shortest, while the 72 intercostals space method was the longest(P<0.01) . Conclusion The feasibility of diagnosing ILD by 14 intercostals space method, 28 intercostals space method and 72 intercostals space method are high, especially the repeatability and consistency of 14 intercostals space method is excellent, and the examination time is the shortest, which can be used preferentially in clinic.
