| 目的 应用声触组织量化（ Virtual touch Tissue Quantification, VTQ）技术测量肝硬化后食管胃静脉曲张（Esophagogastric Varices，EGV）患者“内窥镜下食管胃静脉套扎及硬化治疗”手术前后肝脏、脾脏的剪切波速度(Liver Shear Wave Velocity，LSWV； Spleen Shear Wave Velocity , SSWV )，与内窥镜检查的EGV程度进行对照，以期研究LSWV， SSWV评价肝硬化并发EGV患者的临床干预疗效的价值。方法 应用VTQ在术前测量42例肝硬化并发中、重食管胃静脉曲张患者的SSWV值及LSWV值，并于术后4周再次检测，比较手术前后患者SSWV、LSWV的数值变化，同时记录手术前后患者的内窥镜下食管胃静脉的曲张程度。使用统计学软件SPSS对于检测结果做统计学分析。结果 （1）中、重度EGV患者间的SSWV值不同， SSWV值在术后4周有下降，以下降0.13m/s为临界值判断手术有效性的灵敏度、特异度分别为61.5%，100%；（2）LSWV值在中、重度EGV患者间以及术前、后均无明显差异；（3）EGV 的Sarin分型间SSWV、LSWV值无统计学意义。结论 VTQ获得的SSWV在评估中、重度EGV患者临床干预的诊疗中有较好的应用价值。 |

| Objective To measure the shear wave velocities of liver and spleen before and after endoscopic esophagogastric vein ligation and sclerotherapy in patients with esophagogastric varices (EGV)after liver cirrhosis by using VTQ (Virtual touch Tissue Quantification), and to compare with the degree of EGV examined by endoscopy in order to study the shear wave velocities of liver and spleen (LSWV,SSWV). The value of shear wave velocity in evaluating the clinical intervention effect of patients with liver cirrhosis complicated with EGV.Methods The SSWV and LSWV values of 42 patients with liver cirrhosis complicated with EGV were measured before and after endoscopic esophagogastric varices ligation and sclerotherapy by VTQ. The SSWV and LSWV values were measured again 4 weeks after operation, and the changes of SSWV and LSWV before and after operation were compared. At the same time, the degree of EGV was recorded before and after operation. Statistical software SPSS was used for statistical analysis of the test results.Results (1) The SSWV values of moderate and severe EGV were different. SSWV values decreased 4 weeks after endoscopic esophagogastric varices ligation and sclerotherapy. The sensitivity and specificity were 61.5% and 100% respectively when the critical value was 0.13m/s. (2) There was no significant difference in LSWV value between patients with moderate and severe EGV and before and after operation. (3) Under Sarin's classification of EGV, there was no significant difference in SSWV and LSWV between different types.Conclusion SSWV measurement by VTQ technique has a good application value in evaluating the clinical intervention of patients with moderate to severe esophagogastric varices. |
