目的 比较三维电脑断层扫描(3D-CT)和三维核磁共振(3D-MRI)在诊断关节盂骨缺损中的应用。方法 选取2017年12月至2018年10月在我院就诊的21例肩关节不稳患者纳入本次研究。所有患者均接受3D-CT和3D-MRI检测。比较通过3D-CT和3D-MRI 图像分析出的关节盂面积、骨缺损面积以及骨缺损程度,并对两种不同检查方法所得的测量结果的关系进行分析。结果 根据3D-CT图像测量的最佳拟合圆的表面积较大于3D-MRI图像,但差异无统计学意义(P>0.05)。3D-CT和3D-MRI图像测量骨缺损情况比较,表面积和骨缺损程度差异均无统计学意义(P>0.05)。经线性回归分析可见,3D-CT和3D-MRI测的的最佳拟合圆表面积的回归系数为1.063(R2=0.784);而测得骨缺损表面积之间的回归系数为1.049(R2=0.879)。结论 3D-MRI和3D-CT在诊断肩关节关节盂骨缺损的作用相当,3D-MRI可能可以代替3D-CT为肩关节不稳定的患者在术前进行更加全面、无辐射的检测和评估。 |

Objective: To compare the application of three dimensional computerized tomography (3D-CT) and three-dimensional magnetic resonance imaging (3D-MRI) in the diagnosis of glenoid bone loss. Methods: A total of 21 patients with shoulder instability in our hospital from December 2017 to October 2018 were involved in this study. All of the patients took the examination of both 3D-CT and 3D-MRI. Glenoid surface area, bone loss area and the percentage of bone loss were measured from 3D-CT and 3D-MRI imaging and compared. The correlation of the measurement results from 3D-CT and 3D-MRI was analyzed. Results: The surface area of the best fitting circle in 3D-CT imaging was a little bit larger than it in 3D-MRI imaging, but there was no significant difference (P>0.05). When comparing the bone loss measured between 3D-CT and 3D-MRI, there was no significant difference in the surface area of blood loss and the percentage of glenoid bone loss between two methods (P>0.05). According to linear regression analysis, the regression coefficient of the best fitting circular surface area was 1.063 (R2=0.784) between 3D-CT and 3D-MRI and it of the bone loss surface was 1.049 (R2=0.879). Conclusion: 3D-CT and 3D-MRI demonstrated the similar application value in the diagnosis of glenoid bone loss. 3D-MRI could take the place of 3D-CT to provide a full and non-radiative pre-operative examination in patients with shoulder instability. |
