      目的 以关节镜为“金标准”,研究T1ρ技术对踝关节距骨骨软骨损伤的诊断价值,对比T1ρ、PDWI对早期踝关节软骨损伤诊断的敏感性。方法 研究40例临床诊断为踝关节损伤拟住院行关节镜检查的患者,所有患者均接受术前MR和T1ρ成像检查,随后的1周内行关节镜检查。冠状位测量正常与损伤软骨的T1ρ值,所有病例均记录MR和关节镜结果。结果 T1ρ技术可清晰地显示踝关节距骨软骨,且呈现不同色阶。正常软骨的T1ρ值为(32.66±1.92)ms,随着软骨损伤的加重T1ρ值逐渐增高。Ⅰ-Ⅱ型软骨损伤的T1ρ值无明显差异(P=0.12>0.05),其余各分型软骨的T1ρ值差异均具有统计学意义(P<0.05)。结论 T1ρ技术能够客观的分析踝关节距骨软骨的损伤程度;T1ρ技术对于发现早期软骨损伤非常敏感,较常规PDWI有一定的优势。

      Objective To explore the value of T1ρ in the diagnosis of osteochondral injury of talus in ankle using the arthroscopy as the “gold standard”, and to compare the diagnostic sensitivity of T1ρ and PDWI for early cartilage injury. Methods 40 patients clinically diagnosed ankle joint injures underwent both conventional MR and T1ρ MR imaging preoperative, and then underwent arthroscopy examination within one week. T1ρ values of the normal and damage cartilage were measured on coronal images. All cases were recorded results of MR and arthroscopy. Research T1ρ could show ankle cartilage clearly, and performed different color gradation. T1ρ values of normal cartilage were (32.66±1.92) ms, and increased with aggravation of cartilage injury. The difference between type Ⅰ and type Ⅱ cartilage injuries had no statistical significance(P=0.12>0.05), the differences of other groups all had statistically significant(P<0.05). Conclusion T1ρ technique can objectively analysis the injury degree of talus cartilage through quantitative measurement. T1ρ technique is very sensitive to early cartilage injury and have some advantage compared with conventional MR.
