目的 探讨不同CT扫描参数与IMRA算法效果的相关性。 方法 对植入金属螺钉的仿真胸部体模,采用不同管电压(80-130 kV)以及不同管电流量(20-120mAs)进行扫描,扫描后采用IR算法和IMAR算法分别重建图像,测量金属螺钉邻近肺组织和椎旁软组织的CT值、噪声(CT值SD)及CT值偏差(ΔHU),并进行统计学分析。结果 (1)在相同管电流量(70mAs)扫描下,随着管电压从80kV增加至130KV,图像中金属伪影不断减少,主观评分均值分别为0.83、1.5、2.16,差异有显著性(P<0.05)。肺、椎旁软组织SD和ΔHU随着管电压的增加不断减小,线性回归分析相关性好。(2)在相同管电压(130kV)条件下,随着管电流量的逐渐增加,图像中金属伪影变化不明显,主观评分差异无显著性(P>0.05),肺、椎旁软组织SD和ΔHU变化无规律性,线性回归分析无明显相关性。(3)在低、中、高三组辐射剂量(DLP=100、150、200mGy*cm)下,各组内采用高管电压联合低管电流量扫描图像的金属伪影均明显减少,主观评分及客观指标(SD、ΔHU)差异均有显著性(P<0.05)。与中、高辐射剂量组中110kV/68mAs和80kV/310mAs图像相比较,采用低辐射组中130kV/29mAs扫描后,主观评分增高,肺、椎旁软组织的SD和ΔHU减小。结论 管电压是影响IMAR的主要参数,实际工作中应用高管电压联合低管电流量扫描可以在保证IMAR减少金属伪影效果的前提下,降低患者的辐射剂量。 |

purpose To study the correlation between different parameters of CT scanning and effect of IMAR algorithm. Methods An anthropomorphic chest phantom with spinal screws was scanned at various tube voltage(80-130 kV) and tube current-time(20-120mAs)setting.The data was reconstructed with iterative reconstruction(IR) and Iterative metal artifact reduction (IMAR).The CT values and mean SD of different tissue were measured ,and then was being statistical analyzed. Results (1)In the scanning with same tube current-time(70mAs), as the tube voltage is increased from 80 kV to 130 kV, the image quality is improve for metal artifacts,the mean subjective evaluation scores of image are 0.83、1.5、2.16respectively,the objective index(SD,ΔHU)of the lung parenchyma and the paravertebral soft tissue are decreased with the increase of the tube voltage, the correlations between the noise and the tube voltage of each organization are good. (2) In the scanning with same tube voltage(130kV), with the gradual increase of tube current-time,the improvement of the image quality is not obvious for the metallic artifact, the difference of subjective evaluation scores are no significant. The variations of SD and ΔHU using IMAR algorithm were no regularity, the correlations between the noise and the tube voltage of each organization are poor. (3) In the three groups with low, middle and high radiation dose level, the improvement of the image quality is obvious for the metallic artifact using higher kV combine lower mAs in each group, the variations of subjective evaluation scores and objective index are significant(P <0.05) .Compare with 110kV/68mAs and 80kV/310mAs in middle and high radiation dose group, the subjective evaluation scores are increase and objective index decrease using 130kV/29mAs. Conclusion The tube voltage was the main parameter to influence the effect of IMAR algorithm. It was suggested that using higher kV combine lower mAs can optimize the effect of IMAR, and decrease the radiation dose in the actual work. |
