目的 探讨副鼻窦CT平扫中,器官剂量调制(ODM)技术对眼眶区晶状体的辐射剂量及图像质量的影响。方法 采用前瞻性研究方法,收集65例在GE Revolution CT上行副鼻窦CT平扫的儿童患者,并随机分为A、B两组,A组对照组采用256排探测器常规CT扫描(n=27),设置管电压为100kV,自动管电流120-250mA,噪声指数设为6.00,螺距0.984:1;B组试验组采用256排探测器添加眼眶区ODM技术的CT扫描(n=38),自动管电流&ODM为120-250mA,其他与A组一致。分别记录两组患者眼眶区前、后、左、右(A、P、L、R)四个方向的管电流,并记录剂量长度乘积(DLP)和容积CT剂量指数(CTDIvol);同时,对A、B两组的图像质量进行客观评价。A、B两组年龄、管电流、辐射剂量及客观图像质量的对比采用t检验比较,P<0.05具有明显统计学意义。结果 两组患儿的年龄差异无统计学意义(P>0.05);A、B两组在眼眶前方向的管电流平均值分别为(141.00±26.46)mA和(127.39±17.98)mA,其差异具有统计学意义(t=2.47,P=0.016);眼眶后、左、右方向的管电流均无明显统计差异学。此结果表明ODM技术可以有效减少患者在前方向的管电流,对其他方向的管电流影响不大。A组的CTDIvol和DLP分别为 (14.58±1.89)mGy和(196.43±31.85)mGy*cm,B组为(15.71±0.11)mGy,(210.49±47.78) mGy*cm,其差异无明显统计学意义(P>0.05),结果表明ODM技术并不能有效降低患者眼眶区域扫描所接受的辐射剂量;A、B两组间的图像质量差异无明显统计学意义(P>0.05)。结论 在儿童副鼻窦CT平扫中应用ODM技术,在不影响图像质量的前提下,可以有效降低敏感器官眼眶前方向管电流,进而降低眼眶区域局部辐射剂量,从而达到保护敏感器官晶状体的目的。 |

Objective To explore the effect of organ dose modulation (ODM) technology on the radiation dose and image quality of the crystalline lens in orbit area. Material and Methods Sixty-five children with sinus CT examination were retrospectively enrolled in this study. The patients were randomly divided into 2 groups:A (n=27, conventional CT scan) and group B (n=38, scan with ODM technique). In both groups, 100 kV tube voltage and automatic current modulation were used. The tube currents in different directions (Anterior/Left/Posterior/Right) of orbit were measured and analyzed in the two groups. CTDIvol, DLP and objective image quality were recorded and analyzed as well. Results There was no difference in age between the two groups (P>0.05). The tube currents in anterior direction in group A and group B were (141.00 ±26.46)mA and (127.39 ± 17.98)mA respectively, and the difference was statistically significant (t=2.47, P=0.016). However,the tube currents in L/R/P directions were no statistically significant between two groups (P>0.05). The value of CDTIvol and DLP in group A was respectively (14.58 ±1.89)mGy and (196.43 ± 31.85)mGy *cm. In group B, CDTIvol and DLP were (15.70±2.88)mGy and (210.49 ± 47.78) mGy *cm, the difference has no obviously statistical significance (P>0.05). There was no statistically significant difference in image quality between groups A and B (P>0.05). Conclusion ODM technique can reduce the tube current of orbital region in the front direction, and then reduce the radiation dose in the orbital region that can protect the radiation-sensitive organ. meanwhile maintain the image quality in pediatric sinus CT scanning. |
