【摘要】 目的 探讨3.0T MRI单侧髋关节扫描对圆韧带损伤的诊断价值。方法 回顾性分析41例慢性髋关节疼痛患者的髋关节镜及术前髋关节MRI资料。定义两种MRI扫描方式为:术前MRI扫描方式包括双侧髋关节扫描及单侧髋关节扫描。以关节镜结果为金标准,应用统计软件首先分别比较并分析两种扫描方式与髋关节镜对于检查圆韧带损伤能力的差异 ,然后比较并分析两种扫描方式的诊断效能。结果 41个髋关节中,34例圆韧带损伤,其中4例单髋MRI显示正常;,7例正常,其中1例单髋MRI显示为部分撕裂。MRI资料显示两种扫描方式对圆韧带损伤的敏感度、特异度、阳性预测值、阴性预测值、准确率分别为58.82%(20/34)和88.24%(30/34)、57.14%(4/7)和85.71%(6/7)、86.96%(20/23)和96.77%(30/31)、22.22%(4/18)和60.0%(6/10)、58.54%(24/41)和87.80%(36/41)。单髋扫描的受试者工作曲线下面积大于双髋扫描,且差异具有统计学意义(P=0.02)。结论 3.0T MRI单侧髋关节扫描是诊断圆韧带损伤较好的影像学检查方法。 |

[Abstract] Objective To investigate the diagnostic value of unilateral hip 3.0T MRI in ligamentum teres tears. Methods The hip arthroscopy and preoperative hip MRI data of 41 patients with chronic hip pain were retrospectively analyzed.Preoperative MRI scans include bilateral hip scans and unilateral hip scansTwo scanning methods are defined: bilateral hip and unilateral hip.Taking arthroscopy asAs the aArthroscopy results isare the gold standard, ,aApplication of statistical software was uded to compare and analyze the difference of ligamentum teres tears examination between the two scanning methods and arthroscopy respectivelythe ability of hip arthroscopy to detect the damage of ligamentum teres tearsThe difference between the two scanning modes to the hip arthroscopy in the ability to detect ligament teres were compared,respectively.Then the difference in diagnostic performance between the two scanning modes were analyzed. . Results Among the 41 hip joints, hip arthroscopy confirmed that 34 cases of the ligamentum teres were tears,and 4 cases of unilateral hip MRI showed were normal;, anotherd 7 cases were normal,but one of them ,a case showed partial tear in unilateral hip. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of the two scanning methods for ligamentum teres tears were 58.82% (20/34) and 88.24% (30/34), 57.14%(4/7) and 85.71% (6/7), 86.96% (20/23) and 96.77% (30/31), 22.22% (4/18) and 60.0% (6/10), 58.54% (24 /41) and 87.80% (36/41), respectively. The unilateralunilateral hip scanning mode had a larger area under the curve than the bilateral hip one, and the difference was statistically significant (P= 0.02). Conclusion The 3.0T MRI unilateralunilateral hip scanning is an excellent imaging method for the diagnosis of ligamentum teres tears. |
