| 目的 评价经食管超声心动图(TEE)在肝细胞癌伴下腔静脉癌栓(HCCIVCTT)外科治疗中的应用价值。方法 采用TEE对10例HCCIVCTT患者进行常规的术中检查,确定癌栓位置并分型,术中引导下腔静脉阻断带的放置,术后评价癌栓是否取净。结果 对10例HCCIVCTT患者进行检查发现,癌栓均侵入肝静脉并向下腔静脉延伸,其中1例进入右心房,癌栓形态不一、大小不等、回声不均,阻塞段血管腔内见高速湍流信号。Ⅰ型 4例,Ⅱ型5例,Ⅲ型1例。术中经TEE的引导下Ⅰ型和Ⅱ型患者均于癌栓上方成功放置预阻断带,实时动态监测均未见癌栓断裂、脱落,术后癌栓均已取尽。结论 TEE在HCCIVCTT围手术期发挥重要作用。 |

| Objective To evaluate the value of transesophageal echocardiography (TEE) in the surgical treatment of hepatocellular carcinoma with inferior vena cava tumor thrombus (HCCIVCTT). Methods 10 patients with HCCIVCTT were examined by intraoperative TEE, To determine its classification, to guide the placement of the inferior vena cava barrier band,and evaluate whether the cancer suppositories are net. Results Hepatic vein tumor thrombus intrusion were seen in the 10 cases of HCCIVCTT patients, and extended to the inferior vena cava, of which 1 case into the right atrium. 4 cases of type Ⅰ, 5 cases of type Ⅱ, 1 case of type Ⅲ.The inferior vena cava barrier band were successfully placed above the tumor thrombus by the guiding of Intraoperative TEE in type Ⅰ and Ⅱ patients , tumor thrombus fracture, shedding were not seen in real-time dynamic monitoring, postoperative tumor thrombus have been exhausted. Conclusion TEE plays an important role in HCCIVCTT during perioperative period. |
