      目的 评价基于CT平扫和增强图像的直方图特征是否有助于甲状腺良恶性结节的鉴别诊断。方法 回顾性分析我院经手术后病理证实甲状腺结节患者132例,共140个结节。选取轴位病灶最大层面,采用Mazda软件沿病灶边缘勾画ROI并进行直方图分析,获取9个参数。比较两组数据的差异,并通过ROC曲线分析有统计学差异的灰度直方图参数对甲状腺良恶性结节间鉴别诊断效能。 结果 CT平扫恶性组均值、第10、50、90百分位数高于良性组,差异有统计学意义(P均<0.05);CT增强良性组和恶性组间均值、方差及第1、10、50、90百分位数总体差异均有统计学意义(P均<0.05),恶性组均值、第1、10、50、90百分位数高于良性组,良性组方差高于恶性组。在CT平扫和增强直方图的参数中,第10百分位数AUC最高,为0.68,其鉴别甲状腺良恶性结节的敏感度、特异度分别为74.32%、62.12%。结论 CT直方图分析可作为鉴别甲状腺良恶性结节的重要辅助手段。

      Objective To evaluate whether histogram features based on plain and enhanced CT images are helpful for differential diagnosis of benign and malignant thyroid nodules.Method Retrospective analysis of 132 cases of thyroid nodules confirmed by postoperative pathology in our hospital, a total of 140 nodules.Select the largest level of axial focus, use Mazda software to sketch ROI along the edge of focus and perform histogram analysis to obtain 9 parameters.The differences between the two groups of data were compared, and the statistically significant gray-scale histogram parameters were used to analyze the differential diagnosis between benign and malignant thyroid nodules. Result The mean value of the CT scan, the 10th, 50th, and 90th percentiles were higher than the benign group, and the difference was statistically significant (P<0.05).There were statistically significant differences in the mean, variance, and the first, tenth, 50th, and 90th percentiles between CT enhancement scanning the benign and malignant groups (P<0.05).The average value of malignant group is higher than that of benign group by the first, tenth, fiftieth and ninetieth percentage points, and the variance of benign group is higher than that of malignant group.Among the parameters of CT plain and enhanced histogram, the 10th percentile AUC was the highest, 0.68, and the sensitivity of identifying benign and malignant thyroid nodules was 74.32% and specificity was 62.12%.Conclusion CT histogram analysis can be used as an important auxiliary means to identify benign and malignant thyroid nodules.
