| 目的 观察显微超声造影(SR CEUS)微血管定量参数鉴别胸膜下良、恶性病变(SPL)的价值。方法 前瞻性纳入44例首诊SPL患者,根据病理或随访结果分为良性组11例和恶性组33例。行常规超声、超声造影(CEUS)及SR CEUS,对比组间微血管定量参数差异;绘制受试者工作特征曲线,计算曲线下面积(AUC),评估各参数预测效能。结果 恶性组血管密度(VD)、血流容积(FWVD)、分形维数(FD)、灌注指数及密度分布均低于良性组(P均＜0.05);组间速度分布、速度方差(Vel Var)、方向方差(Dir Var)差异均无统计学意义(P均＞0.05)。上述各单一参数的AUC为0.759~0.959,多参数联合的AUC为0.983。结论 SR CEUS微血管定量参数鉴别良、恶性SPL价值较高。 |

| Objective To explore the value of microvascular quantitative parameters of super resolution contrast-enhanced ultrasound (SR CEUS) for differentiating benign and malignant subpleural pulmonary lesions (SPL). Methods Totally 44 patients with first diagnosed SPL were prospectively enrolled and divided into benign group (n=11) and malignant group (n=33) according to pathology or follow-up results. Conventional ultrasound, contrast-enhanced ultrasound (CEUS) and SR CEUS examinations were performed. Microvascular quantitative parameters were compared between groups. The receiver operating characteristic curve was plotted, and the area under the curve (AUC) was calculated to evaluate the diagnostic efficacy of each parameter. Results Vascular density (VD), flow-weighted vessel density (FWVD), fractal dimension (FD), perfusion index and density distribution in malignant group were lower than those in benign group (all P＜0.05), while no significant difference of velocity distribution, velocity variance (Vel Var), nor directional variance (Dir Var) was found between groups (all P＞0.05). AUC of each parameter alone ranged from 0.759 to 0.959, while of combination of multi-parameter was 0.983. Conclusion Quantitative parameters of SR CEUS microvessels had high value for distinguishing benign and malignant SPL. |
