| 目的 对比不同人体测量指标优化肝脏增强CT碘对比剂剂量的价值。方法 前瞻性纳入172例拟接受肝脏增强CT且对比剂方案相同患者,以肝实质强化差值(ΔHU)=50 HU为标准,对比基于身高(H)、体质量(W)、体质量指数(BMI)、体表面积(BSA)、瘦体重(LBW)及血容量(BV)的预测对比剂剂量,及其与实际剂量的差异。结果 基于H、W、BMI、BSA、LBW及BV的预测对比剂剂量分别为(84.97±4.19)、(56.46±10.15)、(56.47±8.88)、(64.00±6.78)、(56.29±9.14)及(55.39±8.90)ml;基于H的预测对比剂剂量高于,而基于其他人体测量指标、尤其基于W、BMI、LBW及BV的预测对比剂剂量均低于实际剂量(P均<0.001)。男性基于H、W、BSA、LBW及BV的预测对比剂剂量均高于女性(P均<0.001)。结论 基于W、BMI、BSA、LBW及BV进行预测均有助于减少肝脏增强CT碘对比剂剂量,其中基于W、LBW及BV预测可兼顾不同性别。 |

| Objective To compare the value of different anthropometric indices for optimizing the dosage of iodine contrast medium for liver enhanced CT scanning. Methods Totally 172 patients who would undergo liver enhanced CT scanning with a fixed iodine contrast medium injection protocol were prospectively enrolled. Taken 50 HU as the criterion of enhancement difference of hepatic parenchyma (ΔHU), the predicted contrast medium dosages derived from height (H), weight (W), body mass index (BMI), body surface area (BSA), lean body weight (LBW) and blood volume (BV), as well as their discrepancies with the actual administered dosage were calculated and compared. Results The predicted contrast medium dosage based on H, W, BMI, BSA, LBW and BV was (84.97±4.19), (56.46±10.15), (56.47±8.88), (64.00±6.78), (56.29±9.14) and (55.39±8.90) ml, respectively. The predicted dosage based on H was higher, while derived from other anthropometric indices, especially those based on W, BMI, LBW and BV were all lower than the actual dosage (all P<0.001). The predicted contrast medium doses calculated based on H, W, BSA, LBW and BV were all higher in males than those in females (all P<0.001). Conclusion W, BMI, BSA, LBW and BV all helped reduce the iodine contrast medium dose for liver enhanced CT, with W-, LBW- and BV-based formulas accommodating gender differences. |
