| Objective To explore the value of multi-parameter based individualized calculation of iodine contrast agent dosage for improving homogenization of enhanced CT scanning. Methods A total of 278 patients who would undergo non-contrast and triple-phase enhanced abdominal CT examinations were prospectively enrolled and divided into 120 kV group (n=84), 100 kV group (n=103) and 80 kV group (n=91). The dosage of iodine contrast agent was calculated using a mobile application (APP) named "Iodinated Contrast Media Calculator V3.0" based on patient’s gender, age, height, body mass, iodine concentration and tube voltage of CT scanning. Additionally, 58 patients who received non-contrast and triple-phase enhanced abdominal CT with tube voltage of 120 kV and contrast medium protocol of 1.5 ml per kilogram body mass were taken as controls (control group). Liver parenchymal enhancement values were measured in portal venous phase and non-contrast images.Intergroup comparisons were performed, and the measured values were compared with the target enhancement value of 60 HU. The proportion of enhancement value that met diagnostic imaging criteria (≥50 HU) was calculated in each group. Bland-Altman analysis was applied to assess the agreement between measured enhancement value and target value, and coefficient of variation (CV) of enhancement values was calculated for each group. Results The mean enhancement value was (59.12±5.67), (59.47±5.31) and (60.51±5.49) HU in 120 kV, 100 kV and 80 kV groups, respectively, no significant difference was found among these three groups (F=1.556, P>0.05). Among 278 cases in 3 observation groups, enhancement values in 271 cases (271/278, 97.48%) were ≥50 HU, while in 7 cases (7/278, 2.52%) were <50 HU. No significant difference between measured enhancement value and target value was found in all 4 groups (all P>0.05). The mean bias between measured enhancement value and target value in 3 observation groups was 0.30 (95%CI: -0.35, 0.95) HU, with 95% limits of agreement (LoA) ranging from -10.46 to 11.06 HU. In control group, the mean bias was -2.07 (95%CI: -4.64, 0.50) HU, and the 95%LoA ranged from -21.25 to 17.11 HU. CV of enhancement values was 9.59%, 8.92% and 9.07% in 120 kV, 100 kV and 80 kV groups, respectively, all lower than that in control group (15.77%). Conclusion Multi-parameter based individualized calculation of iodine contrast agent dosage protocol could effectively improve homogeneity level of enhanced CT scanning. |
