      目的 观察基于体素内不相干运动-弥散加权成像(IVIM-DWI)的虚拟MR弹性成像(vMRE)评估高原地区正常孕妇孕中、晚期胎盘硬度的价值。方法 回顾性纳入47名接受MR检查的高原地区健康孕妇,包括孕中期19名(孕中期组)和孕晚期28名(孕晚期组)。基于IVIM-DWI获取胎盘各部位虚拟剪切模量(μdiff)、移位表观弥散系数(sADC)及表观弥散系数(ADC)并进行组间比较;采用Pearson相关性分析评估上述参数及胎盘硬度与孕周的线性关系。结果 孕中期组整体胎盘、胎儿侧及母体侧硬度与体积均低于孕晚期组,而sADC与ADC高于孕晚期组(P均<0.05)。胎盘胎儿侧硬度与孕周呈正相关(r=0.369),其sADC(r=-0.386)及ADC(r=-0.448)均与孕周呈负相关(P均<0.05)。结论 基于IVIM-DWI的vMRE可用于无创定量评估高原地区正常孕妇孕中、晚期胎盘硬度。

      Objective To explore the value of intravoxel incoherent motion-diffusion weighted imaging (IVIM-DWI)-virtual MR elastography (vMRE) for evaluating middle and late pregnancy placental stiffness in normal pregnant women at high altitude. Methods A total of 47 healthy pregnant women residing at high altitude who underwent MR examinations were retrospectively enrolled and divided into middle pregnancy group (n=19) and late pregnancy group (n=28). Then virtual shear modulus (μdiff), shifted apparent diffusion coefficient (sADC) and apparent diffusion coefficient (ADC) in different placental regions were obtained based on IVIM-DWI and compared between groups. Pearson correlation analysis was used to explore the linear relationships of placental MRI parameters, placental stiffness and gestational age. Results The stiffness and volume of the whole placenta, as well as fetal and maternal compartment placenta in middle pregnancy group were all lower, while sADC and ADC were both higher than those in late pregnancy group (all P<0.05). The stiffness of fetal compartment placenta showed positive correlation with gestational age (r=0.369), while sADC (r=-0.386) and ADC (r=-0.448) were negatively correlated with gestational age (all P<0.05). Conclusion IVIM-DWI-vMRE could be used to noninvasively quantify middle and late pregnancy placental stiffness in normal pregnant women at high altitude.
