      目的 观察血管铸型联合CT三维重建技术用于定量分析观察妊娠糖尿病(GDM)胎盘血管结构特征的价值。方法 前瞻性纳入75例GDM(GDM组)及75名健康孕妇(对照组);制备胎盘血管铸型并行CT三维重建,测量胎盘血管容积、血管分级数、各级血管直径及血管密度并进行组间比较。结果 GDM组胎盘血管总容积、动脉容积、静脉容积及动、静脉分级数均低于对照组(P均<0.05);GDM组脐静脉2~5级(V2~V5)、脐动脉2~4级(A2~A4)直径及血管总密度、动脉密度、静脉密度亦均低于对照组(P均<0.05);组间1级血管直径差异均无统计学意义(P均>0.05)。结论 利用血管铸型联合CT三维重建能精准量化GDM胎盘血管结构改变。
英文摘要:
      Objective To observe the value of vascular casting combined with CT three-dimensional reconstruction for quantitative analysis of characteristics of placental vascular structure in gestational diabetes mellitus (GDM). Methods A total of 75 GDM pregnant women (GDM group) and 75 healthy pregnant women (control group) were prospectively enrolled. Placental vascular casts were prepared, and three-dimensional reconstruction of CT images were performed. The vessel volume, arterial and venous grading, diameters of each vascular branch, and vascular density were measured and compared between groups. Results The total placental vascular volume, arterial volume, venous volume, arterial grade count and venous grade count in GDM group were lower than those in control group (all P<0.05). The diameters of umbilical vein grades 2—5 (V2—V5) and umbilical artery grades 2—4 (A2—A4), as well as total vascular density, arterial density and venous density in GDM group were lower than those in control group (all P<0.05). No significant difference of diameters of grade 1 blood vessel was found between groups (both P>0.05). Conclusion Vascular casting combined with CT three-dimensional reconstruction could be used to accurately quantify structural changes of placental vessels in GDM.
