| 目的 观察亚实性肺结节(SSN)自然生长规律,以及可用于预测其增长风险的基线CT指标。方法 回顾性纳入140例接受5年以上CT随访的孤立性SSN患者,根据SSN生长趋势分为增长组(n=56)与稳定组(n=84);计算增长组SSN体积倍增时间(VDT)和质量倍增时间(MDT),分析其增长规律。对SSN基线CT定性指标及定量指标进行组间比较,筛选SSN增长与否的独立预测因子。结果 增长组SSN中位VDT为1 510.33天、中位MDT为1 472.67天;其中50.00%(28/56)于随访前3年内、46.43%(26/56)于3~5年间出现增长。组间基线CT所示SSN类型、分叶征、胸膜牵拉征、毛刺征,以及3D长径、体积、质量、平均CT值、紧凑度及球形度差异均有统计学意义(P均<0.05);其中,SSN体积及球形度均为其增长的独立预测因子(P均<0.05)。结论 SSN多具惰性生长特性,部分在3年内即可出现增长;基于基线CT所示SSN体积及球形度可评估其增长风险。 |

| Objective To observe the natural growth pattern of subsolid pulmonary nodule (SSN), and to screen baseline CT indicators for predicting growth risk of SSN. Methods A total of 140 patients with solitary SSN who underwent CT follow-up for ≥5 years were retrospectively enrolled. According to the growth trend of SSN, the patients were divided into growth group (n=56) and stable group (n=84). The volume doubling time (VDT) and mass doubling time (MDT) of SSN were calculated, and the growth pattern of SSN was analyzed in growth group. Baseline CT qualitative and quantitative indicators of SSN were compared between groups, and independent predictors of SSN growth were screened. Results The median VDT of SSN in growth group was 1 510.33 days, and the median MDT was 1 472.67 days. In growth group, the proportion of nodules grew within the first 3 years of follow-up was 50.00% (28/56), and the proportion of those that grew between 3 and 5 years was 46.43% (26/56). Significant differences of baseline CT indicators, including SSN type, lobulation sign, pleural traction sign, spiculation sign, as well as 3D long diameter, nodule volume, nodule mass, mean CT value, compactness and sphericity were observed between groups (all P<0.05), and SSN’s volume and sphericity were both independent predictors of growing or not (both P<0.05). Conclusion SSN were most indolent in growth, some might grow within 3 years. Nodule volume and sphericity showed in baseline CT could be used to predict growth risk of SSN. |
