| 目的 基于4D flow MRI建立健康人群门静脉(PV)系统血流动力学参考值。方法 前瞻性对31名健康志愿者采集PV系统非增强4D flow MRI,利用CVI 42软件获取脾静脉(SV)、肠系膜上静脉(SMV)、脾-肠系膜上静脉汇合处(SMC)、PV主干肝门侧(简称PV主干)、PV左支(LPV)及PV右支(RPV)血流动力学参数,包括血流量、血容量、峰值速度、最大压力梯度及壁面剪切应力;采用±1.96标准差(SD)或P2.5~P97.5百分位数定义其参考值。结果 因4D flow MRI后处理图像显示血管不佳而剔除2名,针对其余29名进行分析。SV、SMV、SMC、PV主干、LPV及RPV血流量参考值依次为0.07~0.71、0.15~0.91、0.43~1.32、0.36~1.40、0.12~0.42及0.06~0.88 L/min,血容量参考值依次为1.02~8.18、0.76~11.14、6.80~14.17、3.31~17.85、1.86~6.29及1.59~11.31 ml,峰值速度参考值依次为9.69~44.73、8.61~40.29、14.52~43.56、14.23~38.29、9.69~38.16及6.06~35.34 cm/s,最大压力梯度参考值依次为0.08~0.58、0.07~0.59、0.16~0.70、0.02~0.56、0.04~0.58及0.04~0.57 mmHg,壁面剪切应力参考值依次为0.01~0.32、0.07~0.22、0.06~0.20、0.05~0.17、0.04~0.26及0.05~0.15 Pa。结论 基于4D flow MRI参数初步建立了健康人群PV系统血流动力学参考值,可为门静脉高压症血流动力学研究提供参考。 |

| Objective To establish hemodynamic reference values of portal venous (PV) system in healthy population based on 4D flow MRI. Methods Thirty-one healthy volunteers were prospectively enrolled, and non-contrast-enhanced 4D flow MRI of PV system were acquired. CVI 42 software was adopted to obtain hemodynamic parameters of PV, including blood flow volume, blood volume, peak velocity, maximum pressure gradient and wall shear stress, of splenic vein (SV), superior mesenteric vein (SMV), spleno-mesenteric confluence (SMC), the hepatic hilar segment of the main PV (abbreviated as main PV), left portal vein (LPV) and right portal vein (RPV). Reference intervals were defined based on ±1.96 standard deviation (SD) or the P2.5—P97.5 percentiles. Results Two subjects were excluded due to poor vascular delineation on post-processed 4D flow MRI, and the rest 29 ones were included for analysis. The reference ranges of blood flow volume for SV, SMV, SMC, main PV, LPV and RPV was 0.07—0.71, 0.15—0.91, 0.43—1.32, 0.36—1.40, 0.12—0.42 and 0.06—0.88 L/min, respectively, of blood volume was 1.02—8.18, 0.76—11.14, 6.80—14.17, 3.31—17.85, 1.86—6.29 and 1.59—11.31 ml, respectively, of peak velocity was 9.69—44.73, 8.61—40.29, 14.52—43.56, 14.23—38.29, 9.69—38.16 and 6.06—35.34 cm/s, respectively, of maximum pressure gradient was 0.08—0.58, 0.07—0.59, 0.16—0.70, 0.02—0.56, 0.04—0.58 and 0.04—0.57 mmHg, respectively, of wall shear stress was 0.01—0.32, 0.07—0.22, 0.06—0.20, 0.05—0.17, 0.04—0.26 and 0.05—0.15 Pa, respectively. Conclusion Hemodynamic reference values of PV system in healthy population were preliminarily established using 4D flow MRI, which might provide references for hemodynamic investigations of portal hypertension. |
