      目的 观察衰减校正(AC)对碲锌镉(CZT)SPECT定量分析心肌血流量(MBF)与心肌血流储备(MFR)的影响。方法 回顾性分析79例接受CZT-SPECT心肌灌注显像的疑诊或确诊冠心病患者,分别将AC处理前、后图像纳入非AC组(n=79)与AC组(n=79),测算左心室(LV)、左前降支(LAD)、左回旋支(LCX)及右冠状动脉(RCA)的静息MBF(rMBF)、负荷MBF(sMBF)及MFR,并进行定量分析。结果 AC组LV、LAD、LCX及RCA的rMBF与sMBF均低于非AC组(P均<0.001),而组间MFR差异均无统计学意义(P均>0.05)。非AC组LV、LAD、LCX及RCA的rMBF、sMBF及MFR均与AC组上述参数测值呈中度正相关(P均<0.001)。结论 AC可致CZT-SPECT定量分析冠状动脉各主要分支MBF结果减低,但对MFR无明显影响。

      Objective To observe the impact of attenuation correction (AC) on cadmium zinc telluride (CZT) SPECT quantitative analysis of myocardial blood flow (MBF) and myocardial flow reserve (MFR). Methods Data of 79 patients with suspected or known coronary heart disease who underwent myocardial perfusion imaging with CZT-SPECT were retrospectively analyzed. Images before AC processing were taken as non-AC group (n=79), while those after AC as AC group (n=79), respectively. Then rest MBF (rMBF), stress MBF (sMBF), as well as MFR of left ventricle (LV), left anterior descending (LAD), left circumflex (LCX) and right coronary artery (RCA) were calculated and quantitatively analyzed between groups. Results rMBF and sMBF of LV, LAD, LCX and RCA in AC group were all lower than those in non-AC group (all P<0.001), whereas no significant difference of MFR was found between groups (all P>0.05). rMBF, sMBF and MFR of LV, LAD, LCX and RCA in non-AC group were moderately positively correlated with those in AC group (all P<0.001). Conclusion AC could reduce MBF in CZT-SPECT quantitative analysis, but had no significant impact on MFR.
