      目的 观察婴儿脊髓拴系综合征(TCS)高频超声表现。方法 回顾性收集经手术确诊的27例TCS婴儿,回顾其TCS高频超声表现。结果 27例TCS婴儿中,术后诊断5例终丝牵张型TCS、4例Currarino综合征及18例神经管缺陷。5例终丝牵张型TCS及4例Currarino综合征脊髓高频超声均表现为单纯终丝牵张;18例神经管缺陷中,4例可见脊膜膨出、6例见脊髓脊膜膨出, 8例见脂肪脊髓脊膜膨出。婴儿TCS高频超声表现为脊髓圆锥位置下移、 形态狭长并向背 侧移位,马尾结构紊乱;终丝牵张型可见终丝增粗(>2 mm)、搏动减弱或消失;神经管缺陷所致脊髓拴系可表现为椎管后缺损、回声中断,椎管内容物于缺损处向后下膨出形成包块。结论 婴儿TCS高频超声特征表现包括脊髓末端内容物位置、形态及搏动性异常,可伴椎管缺损及椎管内容物膨出。

      Objective To observe high frequency ultrasonic manifestations of infant tethered cord syndrome (TCS). Methods Data of 27 infants with TCS confirmed by surgery were retrospectively analyzed, and high frequency ultrasonic manifestations of TCS were observed. Results Among 27 TCS infants, there were 5 cases of tight filum terminale TCS, 4 cases of Currarino syndrome and 18 cases of neural tube defects. High frequency ultrasound detected filum terminale stretched under spinal cord in 5 cases of tight filum terminale TCS and 4 cases of Currarino syndrome. In 18 cases of neural tube defects, meningomyelocele was found in 4 cases, meningomyelocele in 6 cases, while fatty meningomyelocele was noticed in 8 cases. High frequency ultrasonic manifestations of infant TCS included downward displacement, narrow morphology and dorsal displacement of the conus medullaris, structural disorders of the cauda equina, thickened (diameter>2 mm), weakened or disappeared pulsation of filum terminale in tight filum terminale TCS, spinal tethering in neural tube defects, presenting as posterior defects of the spinal canal with interrupted echo, and posterior downward expansion of spinal canal contents from the defects appearing as an encapsulated mass. Conclusion High frequency ultrasonic manifestations of infant TCS included abnormalities in position, shape and pulsation of the spinal cord end, as well as possible spinal canal defects and bulging of spinal canal contents.
