      目的 探讨以几何校正平面回波成像(EPIC)改进颅脑弥散加权成像(DWI)图像质量的价值。方法 前瞻性纳入23例颅脑肿瘤患者及37名健康受试者,采集标准平面回波成像(EPI)-DWI及EPIC-DWI;比较2种图像显示全部60人的额窦、眶直回及颞骨岩部附近脑组织情况、脂肪抑制效果、几何形变程度(不匹配距离)、信噪比(SNR)、对比度噪声比(CNR)及表观弥散系数(ADC)的差异,以及23例颅脑肿瘤病灶的几何形变程度及其SNR、CNR和ADC差异。结果 EPIC-DWI显示额窦及眶直回效果优于EPI-DWI(P均<0.01);2种图像显示颞骨岩部附近脑组织效果差异无统计学意义(P>0.05)。EPIC-DWI脂肪抑制效果优于EPI-DWI(P<0.01)。针对全部60名受检者及23例脑肿瘤患者,EPIC-DWI与T1WI融合图像的不匹配距离显著小于EPI-DWI与T1WI融合图像(P均<0.01);2种图像SNR、CNR及ADC差异均无统计学意义(P均>0.05)。结论 相比EPI-DWI,EPIC-DWI显示解剖结构较好、脂肪抑制更均匀、图像变形更小,使得所获图像质量更佳。

      Objective To explore the value of echo planar imaging geometry correction (EPIC) for improving quality of cerebral diffusion weighted imaging (DWI). Methods Twenty-three patients with brain tumors and 37 healthy subjects were prospectively recruited. Standard echo planar imaging (EPI)-DWI and EPIC-DWI scanning were performed. The displaying degrees of brain structures near the frontal sinus, orbital gyrus and temporal petrous, the fat suppression efficacy, geometric deformation degree (mismatch distance), as well as signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and apparent diffusion coefficient (ADC) values of all 60 subjects were compared between EPI-DWI and EPIC-DWI images. Among 23 patients with brain tumors,the degrees of geometric deformation and SNR, CNR and ADC values of lesions were also compared between EPI-DWI and EPIC-DWI. Results EPIC-DWI was superior to EPI-DWI for visualizing the frontal sinus and orbital gyrus (both P<0.01), while no significant difference of displaying the areas near the temporal petrous between EPI-DWI and EPIC-DWI images (P>0.05). EPIC-DWI demonstrated better fat suppression effect than EPI-DWI (P<0.01). The mismatch distance between the fusion images of EPIC-DWI and T1WI for all 60 subjects and 23 brain tumors patients were both significantly less than that of EPI-DWI and T1WI fusion images (both P<0.01). No significant difference of SNR, CNR nor ADC values of lesions was found between EPI-DWI and EPIC-DWI images (all P>0.05). Conclusion Compared to EPI-DWI, EPIC-DWI displayed anatomical structures better, with more uniform fat suppression and less image distortion, resulting in better imaging quality.
