目的 观察PET/CT超级迭代技术诊断良、恶性孤立性肺结节(SPN)、评估肿瘤侵袭程度及Ki-67表达的价值。方法 纳入53例经病理确诊并接受PET/CT检查的恶性SPN患者(恶性组)及53例良性SPN患者(良性组),采用超级迭代技术获取病灶最大标准摄取值(SUVmax)及平均标准摄取值(SUVmean),以受试者工作特征曲线分析PET/CT参数判断良、恶性SPN的价值;比较不同侵袭程度及表达Ki-67情况的恶性SPN PET/CT参数差异,以Spearman相关性分析评价PET/CT参数与恶性SPN侵袭程度及Ki-67表达的相关性。结果 PET/CT超级迭代技术显示恶性组SUVmax、SUVmean均高于良性组(P均<0.05)。以SUVmax、SUVmean判断良恶性SPN的曲线下面积分别为0.738、0.847。随着肿瘤侵袭程度增加,恶性SPN的SUVmax、SUVmean均呈升高趋势(P均<0.001)。Ki-67阳性SPN的SUVmax、SUVmean均高于阴性者(P均<0.05)。恶性SPN的SUVmax、SUVmean与其侵袭程度、Ki-67表达均呈正相关(r=0.340~0.452,P均<0.001)。结论 PET/CT超级迭代技术所获SUVmax、SUVmean与恶性SPN侵袭程度及Ki-67表达具有一定相关性,可用于判断良、恶性SPN。 |

Objective To observe the value of PET/CT super-iterative technique for diagnosing solitary pulmonary nodules (SPN) and evaluating degree of tumor invasion and Ki-67 expression. Methods A total of 53 patients with pathologically confirmed malignant SPN (malignant group) and 53 with benign SPN (benign group) who underwent PET/CT examination were enrolled. The maximum standard uptake value (SUVmax) and the mean standard uptake value (SUVmean) were obtained with super-iterative technique. The value of PET/CT parameters for diagnosing benign and malignant SPN was analyzed using receiver operating characteristic curve. PET/CT parameters of malignant SPN were compared among different tumor invasion degrees and Ki-67 expression status. Spearman correlation analysis was used to evaluate the correlation of PET/CT parameters and malignant SPN invasion degree and Ki-67 expression. Results SUVmax and SUVmean obtained with PET/CT super-iterative technique in malignant group were both higher than those in benign group (both P<0.05). The area under the curve of SUVmax and SUVmean for diagnosing benign and malignant SPN was 0.738 and 0.847, respectively. With the increase of tumor invasion, SUVmax and SUVmean of malignant SPN tended to increase (both P<0.001). SUVmax and SUVmean of Ki-67 positive SPN were both higher than those of Ki-67 negative SPN (both P<0.05). SUVmax and SUVmean of malignant SPN were positively correlated with its invasion degree and Ki-67 expression (r=0.340-0.452, all P<0.001). Conclusion Being correlated with the invasion degree and expression of Ki-67 of malignant SPN, SUVmax and SUVmean obtained with PET/CT super-iterative techniques could be used to judge malignant or benign SPN. |
