目的 观察分层应变技术评估多柔比星(DOX)干预大鼠左心室心肌功能变化的价值。方法 将24只SD大鼠随机分为2周组、4周组、6周组及对照组,每组6只。对各干预组经腹腔注射DOX,建立大鼠心脏毒性模型。对各组行常规超声心动图及分层应变检查,比较组间及组内各参数差异,观察大鼠心肌病理改变。结果 各组间常规超声心动图参数差异均无统计学意义(P均>0.05)。随DOX累积剂量增加,各干预组心内膜下心肌整体纵向应变(GLSendo)、中层心肌整体纵向应变(GLSmid)、心外膜下心肌整体纵向应变(GLSepi)呈阶梯下降趋势,但仅6周组GLSendo低于对照组(P<0.05),而组间GLSmid、GLSepi差异均无统计学意义(P均>0.05);各组内心肌整体纵向分层应变均存在梯度特征,即GLSendo>GLSmid>GLSepi,差异均有统计学意义(P均<0.05)。病理结果显示,随DOX累积剂量增加,大鼠心肌细胞空泡样变、心肌间质纤维化逐渐加重,以心内膜下心肌最为明显。结论 根据分层应变参数、尤其GLSendo可早期、敏感地识别DOX干预大鼠左心室功能改变。 |

Objective To observe the value of stratified strain technique for evaluating the left ventricular myocardial function in rats intervened with doxorubicin (DOX). Methods Totally 24 rats were equally randomized into 2-week group, 4-week group, 6-week group and control group (each n=6). Rat models of DOX induced cardiotoxicity was established through intraperitoneal injection of DOX. Then all rats underwent conventional echocardiography and ultrasound using stratified strain technique, and the parameters were compared among groups and within groups. Finally, rat hearts were harvested, and pathological examinations were performed. Results No significant difference of conventional echocardiography parameters was found among groups (all P>0.05). As the cumulative dose of DOX increasesd, stepwise downward trends of global longitudinal strain of endo-myocardium (GLSendo), global longitudinal strain of mid-myocardium (GLSmid) and global longitudinal strain of epi-myocardium (GLSepi) were observed among 2-week, 4-week and 6-week groups. No significant difference of GLSmid nor GLSepi was found among groups (both P>0.05), while significant difference of GLSendo was detected between 6-week group and control group (P<0.05). There were gradient features of the global longitudinal layered strain of the myocardium, i.e. GLSendo>GLSmid>GLSepi within groups, and the differences were statistically significant (all P<0.05). Pathological results showed that the increased accumulated doses of DOX aggravated vacuolization of cardiomyocytes and deposition of collagen fibers, especially in the endocardial layer. Conclusion Stratified strain parameters, especially GLSendo, could be used to early and sensitively detect changes of left ventricular function in DOX intervened rats. |
