      目的 观察双层探测器光谱CT虚拟单能量成像(VMI)联合去除植入物金属伪影(O-MAR)用于减少置换髋关节假体金属伪影的价值。方法 收集20例接受髋关节双层探测器光谱CT扫描的全髋关节置换术后患者,基于重建CT数据获得常规混合能量图(PI)、加入O-MAR的PI(MAR图)、70~200 keV VMI及其与O-MAR融合图(VMI-MAR图);计算低密度和高密度伪影指数(AI)及臀大肌和膀胱噪声值(SD),并针对伪影严重程度及假体周围组织清晰度进行主观评分;比较不同图像AI、SD和主观评分差异,评估VMI联合O-MAR去除金属伪影的价值。结果 VMI及VMI-MAR图中,多数AI及SD随能级升高而下降,且能级越高越趋于平稳;VMI中,部分AI及SD随能级升高而先降低后升高;AI和SD均于190 keV VMI-MAR图中最低,VMI-MAR图主观评分最高。结论 双层探测器光谱CT VMI联合O-MAR有助于去除髋关节假体金属伪影。

      Objective To observe the value of virtual monoenergetic image (VMI) combined with orthopedic metal artifact reduction (O-MAR) for reducing metal artifacts of hip prosthesis based on dual-layer detector spectral CT. Methods Twenty patients who underwent total hip joint replacement and dual-layer detector spectral CT scanning were enrolled. Based on CT data, conventional polyenergetic image (PI), PI with O-MAR added (MAR image), 70-200 keV VMI and the fusion with O-MAR (VMI-MAR image) were reconstructed. Then low and high density artifact index (AI),gluteus maximus and bladder noise values (SD value) were calculated, and subjective scores were evaluated according to the severity of artifacts and the clarity of tissue around prosthesis. AI, SD, and subjective scores were compared among different kinds of images, and the value of VMI combined with O-MAR for reducing metal artifacts was analyzed. Results On VMI and VMI-MAR images, most AI and SD decreased with the increase of energy levels, and the higher the energy level, the more stable it tended to be. On VMI, some AI and SD decreased firstly and then increased with the increase of energy levels. AI and SD were both the lowest on 190 keV VMI-MAR image. VMI-MAR image had the highest subjective score. Conclusion VMI combined with O-MAR was helpful for reducing metal artifacts of hip prosthesis based on dual-layer detector spectral CT.
