      目的 探讨超高b值弥散加权成像(HBV-DWI)评估促红细胞生成素(EPO)治疗肾缺血再灌注损伤(IRI)效果的价值。方法 将18只健康新西兰大白兔随机均分为IRI+EPO组、IRI组及对照组,每组各6只。对IRI+EPO组于左肾蒂夹闭再灌注后立即腹腔注射EPO(3 000 U/kg体质量),IRI组在相同时间注射等量生理盐水;对照组除未夹闭左肾蒂外均同IRI组。于再灌注48 h后行MR扫描,测量左肾皮质(CO)及外髓(OM)超高b值表观弥散系数(ADCuh);行免疫组织化学染色,测量水通道蛋白(AQP)-1、AQP-2累积光密度(IOD)值;以Pearson相关分析评价左肾CO及OM的ADCuh与免疫组织化学指标的相关性。结果 IRI+EPO组及IRI组左肾CO、OM的ADCuh和AQP-1、AQP-2 IOD值均低于对照组(P均<0.05);IRI+EPO组左肾CO的ADCuh和AQP-1 IOD值、OM的ADCuh、AQP-1和AQP-2 IOD值均高于IRI组(P均<0.05)。左肾CO的ADCuh与AQP-1、AQP-2 IOD值均呈正相关(r=0.756、0.819,P均<0.01);左肾OM的ADCuh与AQP-1、AQP-2 IOD值均呈正相关(r=0.720、0.848,P均<0.01)。结论 HBV-DWI可无创评估EPO治疗兔肾IRI效果。

      Objective To observe the value of ultra-high b value diffusion weighted imaging (HBV-DWI) in evaluating therapeutic effect of erythropoietin (EPO) for renal ischemia reperfusion injury (IRI). Methods Eighteen healthy New Zealand white rabbits were randomly divided into IRI+EPO group, IRI group and control group (each n=6). Rabbits in IRI+EPO group underwent intraperitoneally injection of EPO (3 000 U/kg body mass) immediately after left renal pedicle clamp reperfusion, while those in IRI group were injected the same amount of physiological saline at the same time, and for rabbits in control group, the managements were the same as in IRI group except unclipped left renal pedicle. Forty-eight hours after reperfusion, MR scanning was performed, ultra-high b value apparent diffusion coefficient (ADCuh) of the left renal cortex (CO) and outer medulla (OM) were measured. Then immunohistochemical stainings were used to measure the integral optical density (IOD) of aquaporin (AQP)-1 and AQP-2. Pearson correlation analysis was applied to evaluate the correlation of ADCuh in CO and OM of left kidney and immunohistochemical indexes. Results ADCuh, AQP-1 and AQP-2 IOD values of CO and OM of left kidney in IRI+EPO group and IRI group were all lower than those in control group (all P<0.05). ADCuh and AQP-1 IOD values of CO, ADCuh, AQP-1 and AQP-2 IOD values of OM in IRI+EPO group were all higher than those in IRI group (all P<0.05). ADCuh in CO of left kidney was positively correlated with IOD values of AQP-1 and AQP-2 (r=0.756, 0.819, both P<0.01), while ADCuh in OM of left kidney was positively correlated with IOD values of AQP-1 and AQP-2 (r=0.720, 0.848, both P<0.01). Conclusion HBV-DWI could be used to noninvasively evaluate the effect of EPO for treating renal IRI in rabbits.
