目的 观察产前超声透明隔腔水平横切面用于诊断胼胝体发育不全(ACC)的价值。方法 纳入26胎ACC胎儿(ACC组),以90胎正常胎儿作为对照组。根据对照组颅脑容积数据对透明隔腔水平横切面进行再认识,观察其中胼胝体压部声像图特征并进行组间比较;分析ACC组内低回声带消失或前移率与常规间接征象(侧脑室扩张、透明隔腔变小或消失)的检出率的差异。结果 透明隔腔水平横切面于对照组90胎均显示胼胝体压部,表现为韦氏腔后方低回声带,横跨大脑纵裂与左右大脑半球相连;于ACC组25胎(25/26,96.15%)见低回声带消失或前移,高于超声对常规间接征象的检出率(73.08%,P<0.05)。结论 产前超声透明隔腔水平横切面显示低回声带消失或前移可用于诊断胎儿ACC。 |

Objective To observe the value of prenatal horizontal cross-section ultrasound of septum pellucidum cavity for diagnosing agenesis of corpus callosum (ACC). Methods A total of 26 fetuses with ACC were enrolled (ACC group), while 90 normal fetuses were randomly selected as control group. The horizontal cross-section of septum pellucidum cavity was re-recognized based on fetal cranial volume data of control group, the features of splenium of corpus callosum on the section were observed and compared between groups. The positive rate of hypoechoic cord disappearance or forward movement of ACC group were compared with the detection rate of conventional indirect signs (lateral ventricular dilation, shrinking or disappearance of the hyaline septum). Results The horizontal cross-section of fetal septum pellucidum cavity showed splenium of corpus callosum in all 90 fetuses of control group, presented as a hypoechoic cord located behind the Wechsler cavity across the cerebral longitudinal fissure and connected to the left and right cerebral hemispheres, which disappeared or moved forward in 25 fetuses (25/26, 96.15%) of ACC group, higher than the detection rate of conventional indirect signs (73.08%, P<0.05). Conclusion Disappeared or shifted forward septum pellucidum cavity showed on prenatal horizontal cross-section ultrasound could be used to diagnose fetal ACC. |
