目的 以定量脑电图(QEEG)联合动脉自旋标记成像(ASL)观察成人弱视患者脑功能及血流灌注改变。方法 对19例弱视患者(弱视组)及同期23名正常对照者(HC组)行QEEG和MR检查,并对弱视组行中文版低视力者生活质量量表(CLVQOL)评分;比较组间各脑区QEEG参数功率谱密度(PSD)及3D-pCASL定量参数,包括平均脑血流量(mCBF)、动脉通过时间(ATT)及脑血容量(CBV)的差异,并分析弱视组脑血流灌注参数与CLVQOL评分的相关性。结果 相比HC组,弱视组双侧枕叶θ、α、β、γ频段PSD,左顶叶θ、β、γ频段PSD,右顶叶δ、θ、β、γ频段PSD,左后颞叶γ频段PSD,右后颞叶δ、α、β、γ频段PSD均增大(P均<0.05);其余PSD组间差异均无统计学意义(P均>0.05)。相比HC组,弱视组右侧基底节尾状核腹侧及背侧、伏隔核mCBF均升高,右侧中央后回123区、右侧岛叶颗粒区、左侧岛叶无颗粒区腹侧及右侧基底核苍白球ATT均降低,右侧基底节尾状核腹侧CBV升高(FDR校正P均<0.05)。成人弱视患者左侧枕叶V5/MT+区ATT与CLVQOL-P3评分呈正相关(rs=0.440,P=0.003)。结论 成人弱视患者顶枕叶及后颞叶等局部脑区功能发生改变,基底核区等局部脑区血流灌注存在代偿性改变。 |

Objective To observe the changes of brain function and blood flow perfusion in adult amblyopia patients using quantitative electroencephalography (QEEG) combined with arterial spin labeling (ASL) technology.Methods Totally 19 patients with amblyopia (amblyopia group) and 23 healthy controls (HC group) underwent QEEG and MR examinations during the same period. Patients in amblyopia group were scored with Chinese version of the low vision quality of life questionnaire (CLVQOL). QEEG parameter, i.e. power spectral density (PSD), and 3D-pCASL quantitative parameters, including the mean cerebral blood flow (mCBF), arterial transit time (ATT)and cerebral blood volume (CBV) were compared between groups. The correlations of cerebral blood flow perfusion parameters and CLVQOL scores of amblyopia patients were analyzed.Results Compared with those of HC group, θ, α, β and γ band PSD of bilateral occipital lobes, θ, βand γ band PSD of left apical lobe, δ, θ, β, γ band PSD of right apical lobe, γ band PSD of left posterior temporal lobe, δ, α, β and γ band PSD of right posterior temporal lobe increased in amblyopia group (all P<0.05), while no significant difference of PSD of other lobes was found between groups(all P>0.05). Compared with those of HC group, mCBF of ventral and dorsal parts of the right basal ganglion caudate nucleus and nucleus accumbens increased, ATT of 123 region of right posterior central gyrus, granular region of the right insula, ventral region of the left insula and globus pallidus of right basal ganglion decreased, while CBV of ventral part of right basal ganglion caudate nucleus increased in amblyopia group (FDR correction, all P<0.05). ATT of V5/MT+ area of the left occipital lobe in amblyopia group was positively correlated with CLVQOL-P3 score (rs=0.440, P=0.003).Conclusion Functional changes in local brain regions such as parietal occipital lobe and posterior temporal lobe, as well as blood flow perfusion compensatory changes in local brain regions such as basal ganglia existed in adult amblyopia patients. |
