目的 观察以容积CT剂量指数(CTDIvol)与体型特异性剂量估算(SSDE)所评估的儿童CT辐射剂量差异,分析CTDIvol和剂量长度乘积(DLP)对于儿童CT辐射管控的价值。方法 回顾性分析3 247例患儿,其中1 238例接受头颅CT检查,1 152例接受胸部CT检查,857例接受腹盆部CT检查;记录年龄、CTDIvol及DLP,计算有效直径(De)、水当量直径(Dw)、转换因子(f)及基于Dw的SSDE (SSDEDw)。比较CTDIvol与SSDEDw在不同年龄段儿童CT检查中的差异,观察其评估CT辐射剂量的效能;建立CTDIvol、DLP、f与年龄及De的回归模型,观察其用于儿童CT辐射管控的价值。结果 头颅、胸部及腹盆部CTDIvol与SSDEDw差异均有统计学意义(P均<0.001)。随年龄和De增高,CTDIvol和DLP增加(R2为0.76~0.93)而f减小(R2为0.74~0.99)。结论 相比CTDIvol,以SSDEDw评估儿童CT辐射剂量更为准确;基于年龄和体型估算CTDIvol及DLP有利于更好地实施儿童CT辐射管控。 |

Objective To analyze the difference of CT radiation dose in children estimated based on volume CT dose index (CTDIvol) and size-specific dose estimate (SSDE), and to observe the value of CTDIvol and dose length production (DLP) for radiation control of CT examination in children. Methods Data of 3 247 children were retrospectively analyzed, including 1 238 cases who underwent head CT examination, 1 152 cases who underwent chest CT examination and 857 cases who underwent abdominal and pelvic CT examination. The children's age, CTDIvol and DLP were recorded, and the effect diameter (De), water equivalent diameter (Dw), conversion factor (f) and SSDE were calculated based on Dw (SSDEDw). The differences of CTDIvol and SSDEDw in CT examinations of children with different ages were observed, and the value of CTDIvol and SSDEDw for evaluating CT radiation dose were explored. The regression models of CTDIvol, DLP, f with age, De were established, and it's value for CT examination was evaluated. Results There were significant differences of CTDIvol and SSDEDw among head, chest and abdomen-pelvis CT (all P<0.001). With the increase of age and De, CTDIvol and DLP increased (R2 ranged from 0.76 to 0.93), while f decreased (R2 ranged from 0.74 to 0.99). Conclusion Compared with CTDIvol, SSDEDw could be used to accurately evaluate radiation dose of CT in children. Estimating CTDIvol and DLP based on age and body type was helpful to radiation control of CT in children before examination. |
