目的 对比干燥综合征(SS)干眼病与普通干眼病泪腺超声表现。方法 对33例SS干眼病(66眼,SS干眼组)、30例普通干眼病患者(60眼,普通干眼组)及35名健康人(70眼,对照组)行泪腺二维超声、多普勒超声及超声弹性成像,比较3组泪腺长径、短径和基于泪腺回声的风湿病临床试验结果指标(OMERACT)评分,以及泪腺动脉收缩期峰值流速(PSV)、舒张末期流速(EDV)、阻力指数(RI)、血流评分和应变比(SR)及杨氏模量平均值(Emean)的差异。结果 SS干眼组泪腺长径和短径均长于、OMERACT评分均高于普通干眼组及对照组(P均<0.05);普通干眼组泪腺长径和短径与对照组差异均无统计学意义(P均>0.05),OMERACT评分则高于对照组(P<0.05)。SS干眼组泪腺动脉PSV、RI及血流评分均高于普通干眼组及对照组,其EDV高于普通干眼组而低于对照组(P均<0.05);普通干眼组泪腺动脉PSV及EDV均低于对照组而RI高于对照组(P均<0.05)。SS干眼组泪腺SR及Emean均高于普通干眼组及对照组,普通干眼组泪腺SR及Emean高于对照组(P均<0.05)。结论 SS干眼病与普通干眼病泪腺超声表现有所差异,有助于诊断和鉴别诊断。 |

Objective To compare the ultrasonic manifestations of lacrimal gland in patients with Sjögren syndrome (SS) dry eye disease or ordinary dry eye disease. Methods Thirty-three patients with SS dry eye disease (66 eyes, SS dry eye group), 30 patients with ordinary dry eye disease (60 eyes, ordinary dry eye group) and 35 healthy individuals (70 eyes, control group) were enrolled. Two-dimensional ultrasound, Doppler ultrasound and elastography ultrasound of the lacrimal glands were performed. The long and short diameter of lacrimal gland, the rheumatology clinical trial (OMERACT) scores based on lacrimal gland echogenicity, as well as artery peak systolic velocity (PSV), end diastolic velocity (EDV), resistive index (RI), blood flow score, strain ratio (SR) and mean Young's modulus (Emean) were compared among groups. Results The long and short diameter of lacrimal glands in SS dry eye group were longer than those in ordinary dry eye and control group, respectively, while OMERACT scores in SS dry eye group were higher than those in ordinary dry eye and control group (all P<0.05). The long and short diameter of lacrimal glands group were not significantly different between ordinary dry eye group and control group (both P>0.05), while OMERACT scores in ordinary dry eye group were higher than that in control group (P<0.05). The artery PSV, RI and blood flow scores of lacrimal gland in SS dry eye group were higher than those in ordinary dry eye group and control group, while EDV was higher than that in ordinary dry eye group but lower than that in control group (all P<0.05). The lacrimal gland artery PSV and EDV in ordinary dry eye group were lower than those in control group, whereas RI was higher than that in control group (all P<0.05). SR and Emean of lacrimal gland in SS dry eye group were higher than those in ordinary dry eye group and control group, and SR and Emean of lacrimal gland in ordinary dry eye group were higher than those in control group (all P<0.05). Conclusion Ultrasonic manifestations of lacrimal gland in patients with SS dry eye and ordinary dry eye disease were different, hence being helpful to diagnosis and differential diagnosis. |
